    Diagnostic Contribution of Postmortem Needle Biopsies in Neonates

    Objective: We examined the contribution of neonatal postmortem needle biopsy in circumstances of autopsy denial where magnetic resonance imaging cannot be performed. Material and Method: 247 postmortem needle biopsy specimens of 76 neonatal cases who died in the neonatal intensive care ward of a tertiary hospital between 2005 and 2010 and where the family did not give permission for an autopsy were retrospectively evaluated. Results: 90 needle biopsy attempts (36.4%) were unsuccessful among the 247 biopsies. Liver needle biopsies were found to yield the most valuable diagnostic contribution. A total of 53 liver biopsies provided clinical information that contributed to the diagnosis. Conclusion: The postmortem needle biopsy should be applied in all cases in which standard biopsy cannot be performed. Success rates and level of information gained by extrahepatic needle organ biopsies in neonates were found to be quite low whereas needle biopsies of the liver yielded valuable results. We believe it is more appropriate to perform percutaneous postmortem biopsies solely on the liver in neonates.

    Frequency of Celiac Disease in Attention-deficit/Hyperactivity Disorder

    Objective: Although it is well known that celiac disease (CD) is associated with neurologic disorders, association with psychiatric problems is not well defined. In this report, we aimed to detect CD prevalence in patients with attention-deficit hyperactivity disorder (ADHD). Methods: A total of 362 patients between the ages 5 and 15 years with the diagnosis of ADHD according to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR) diagnostic criteria and 390 sex- and age-matched healthy children were included in the present study. Serum levels of tissue transglutaminase (tTg) immunoglobulin (Ig) A and IgG were studied in both groups. Serum IgA levels were also studied in patients with positive tTG IgG for the exclusion of selective IgA deficiency. Endoscopic duodenal biopsy was provided in seropositive patients, whose parents approved the procedure. Biopsy samples were evaluated according to Marsh-Oberhuber classification. Results: tTg IgA was positive in 4 patients with ADHD (1.1%). Endoscopic duodenal biopsy was suggestive of CD in one of them (0.27%). tTg IgA was positive in 3 of control group patients (0.8%). Duodenal biopsy of the only patient from control group, who underwent upper gastrointestinal endoscopy, revealed normal intestinal mucosa. Conclusions: The seropositivity rates for CD were found similar in ADHD and control groups. Thus, neither routine screening for CD nor empirical recommendation of gluten-free diet seems necessary in children with ADHD.

    Jeune sendromu; diğer adıyla ‘Asfiksik Torasik Displazi’ (ATD) daralmış göğüs kafesi, ekstremite anomalileri, solunum yetmezliği ve kayda değer yenidoğan mortalitesi ile karakterize otozomal resesif kalıtım gösteren nadir bir kondrodisplazidir. Bu sendrom bulunan çocukların çoğu şiddetli solunumsal yetmezlik gösterir ve erken süt çocukluğu döneminde ciddi ventilatör desteğine ihtiyaç duyabilir. Sendromun renal displazi, retinitis pigmentosa, hepatik ve pankreatik fibrozu içerebilen geniş bir klinik spektrumu mevcuttur. Doğum öncesi akondroplazi tanısı bulunan ve doğum sonrası ciddi solunumsal yetmezlik gösteren ve Jeune sendromu tanısı konulan bir hastayı sunduk.

    Long-term benefits of breastfeeding

    Exclusively breastfeeding during the first six months of life is a worldwide accepted mode of feeding infants. Breast milk not only provides the needs of the infant for appropriate growth and development, but also lowers the mortality and morbidity by protecting against infections. It may also have some benefits for the following years and even for the adulthood. Numerous studies conducted on the late effects of breastfeeding reported sometimes inconsistent findings. We hope that review including last scientific knowledge would be helpful for physicians who deal with pregnants, newborns and infants to remember the benefits of breastfeeding.     Malnutrition and Obesity in Children With ADHD

    Objective: Low threshold of dissatisfaction and frustration in child and adolescent patients with ADHD lead to changes in dietary pattern with contribution to the development of obesity and other nutritional disorders. Methods: A total of 362 patients with the diagnosis of ADHD and 390 children without any chronic disease were included to the study. We evaluated the anthropometric measurements for all cases. Results: The overweight/obese cases according to weight for height (WFH) and body mass index scores were more likely in ADHD cases than control group. Malnourished cases according to WFH were more common in ADHD cases. Patients with weight standard deviation scores between <-2 and >2 were significantly higher in ADHD cases than the control group patients. Conclusion: Frequency of malnutrition and overweight/obese patients was found higher in ADHD cases compared with control group, and this situation suggested that ADHD is a risk factor for the development of eating disorders.

    Usage of Turkish language in medical literature and frequent errors

    Language is an instrument for handing down the thoughts, scientific acquirements, poetic and literary products. The initial efforts for determining suitable Turkish counterparts against foreign words and terms have began during the Republican Era. Mustafa Kemal Ataturk, with an educational vision beyond his epoch, has promoted Turkish language as a scientific language instead of Persian and Arabic languages thus conquered our language's independence. In the information age which we are living today, the communities should improve and support their national language of science in order to perform the existing data and to establish every individual to reach the scientific evaluations. The frequently-encountered cases in medical literature that are unappropriate for Turkish language are classified as follows: the foreign terms and words used in spite of an existing Turkish counterpart, ignoring methods for derivation of Turkish terms, translation mistakes, faults in spelling, words denoting different meanings with similar way of writings. The usage of Turkish terms within the academic level, thesis of proficiency and literary products and maintaining writing skills suitable for the Turkish language spelling rules are important for our language to become a scientific language, and pioneering and becoming a precedent efforts are required (Turk Arch Ped 2011; 46: 186-90)
