Öğe Assessment of levels of knowledge of breastfeeding counseling of health personnel working in primary health care institutions(2019) Kayhan Tetik, Burcu; Aksoy Derya, Yeşim; Uçar, Tuba; Coşkun, Ebru İnci; Felek, Selma; Citil, Semsettin; Sener, KemalAbstract: Aim: The aim of this study is to evaluate the knowledge level of breast milk and breastfeeding of health professionals working in primary health care institutions in Malatya. Material and Methods: A Cross-sectional Study’s survey prepared by using the Handbook of Breastfeeding Counseling translated by Child and Adolescent Reproductive Health Branch of the Ministry of Health of Turkey. 778 people were contacted by the interviewers by visiting the primary health care institutions in Malatya city center and districts, directly. All data was evaluated with SPSS 22 software and p<0.05 was accepted as statistically significant. Results: The mean age of the participants was 36.15 ± 8.57 years; 77.6% of them were women; 49.1% were working in Family Health Center. It was determined 80.1% of family physicians and 84.5% of midwives / nurses have been consulting about breast milk / breastfeeding in daily practice. When the respondents’ answers to the statements of knowledge about breast milk/breastfeeding counseling were assessed, there was no significant difference between the answers given by the family physicians and midwives/ nurses (p> 0.05). The participants gave wrong answers to 40% of the questions about conditions that require practical support such as insufficient breastmilk and the conditions related to the breasts. Conclusion: In our study we observed that basic subjects were well known by healthcare personnel whereas there is a serious lack of information, especially in daily practice, which should be constantly updated. We think that the training of breastfeeding and breastfeeding counseling should be updated periodically and especially emphasis should be given to new developments and practical information.Öğe Çok düşük B-hCG düzeylerinde rüptüre tubal gebelik: olgu sunumu(2016) Duz, Senem Arda; Eraslan, Sevil; Coşkun, Ebru İnciÖz: Ektopik gebelik uterin kavitenin dışında oluşan gebeliktir. Sıklıkla tubada olmak üzere abdominal kavite, over ve servikste görülebilir. Ruptüre ektopik gebeliklere bağlı anne ölümleri günümüzde transvajinal sonografi ve serum ?-hCG (beta human koryonik gonadotropin) ölçümleri ile oldukça azalmıştır. Ancak ektopik gebeliğin kliniği ile ?hCGdüzeyi arasında bir korelasyon yoktur. Çok düşük ?-hCG düzeylerinde ektopik gebelik ruptürü gelişen bir olguyu sunmaktayız. Adet rötarı ile başvuran 29 yaşındaki nullipar hastanın serum ?-hCG düzeyi 1051 mIU/ml olup ultrasonografide tubal gebelik düşünülmüş, akut batın bulguları olmaması ve 3. gün serum ?-hCG düzeyi 727 mIU/ml olması üzerine haftalık takibe alınmıştır. Son vizitinden 2 gün sonra şiddetli alt karın ağrısı ile başvurdu. ?-hCG düzeyi 39.8 mIU/ml idi. Ultrasonografide abdomende yaygın mai izlendi, acil laparotomi, sağ salpenjektomi yapıldı. ?-hCG düzeyleri düşük ve azalmakta olsa bile rüptür ihtimali her zaman akılda tutulmalı ve hastalar da bu konuda bilgilendirilerek takip edilmelidir.Öğe The Effects of Serum Neurotensin-C Levels on Insulin Resistance in Polycystic Ovary Patients(2016) Kaplan, Süreyya; Yılmaz, Ercan; Coşkun, Ebru İnci; Çıplak, Barış; Kırıcı, PınarAbstract: Aim: Neurotensin has a role in the onset of diabetes mellitus. In this study, we aim tocompare serum neurotensin levels between obese and non-obese patients with PCOS.Materials and Methods: Patients were divided into four groups. We measured andcalculated the following parametres: age, BMI, biochemical and hormonal profiles, andserum neurotensin levels. The results were compared within the groups. As statisticalmethods, we used the chi-square test and the Mann-Whitney-U test.Results: We found certain differences between the PCOS patients and control groups.Mean values of age, FGS, and LH levels were higher than those of the control groups. LHlevels were higher in the non-obese PCOS patients than the non-obese control grouppatients. Weight, BMI, LDL, and triglycride levels were also higher in the obese PCOSpatients than the non-obese PCOS patients. HOMA-IR values were found to be highest inthe obese PCOS patients. There was no significant difference between the groups in termsof NT. Evaluating the results, we observed that NT levels were similar in the non-obesePCOS patients (NT: 0,67±0,709) and obese control group patients (NT: 0,66±1,47). Similarly,NT levels were quite similar in the obese PCOS patients (NT: 0,43±0,362) and non-obesecontrol patients (NT: 0,47 ±0,406).Conclusion: It was determined that lipid profile and HOMA-IR values are higher in patientswith PCOS. Also, in non-obese PCOS patients and obese control group, the lipid profile,HOMA-IR values, fasting glucose and fasting insulin levels are determined to be significantlyhigher than the values of the non-obese control group. The sociodemographic characteristics and complete blood counts of the patients who admitted to the pregnancy outpatient clinic were included in the study and data obtained from the files registered in the automation system. Evaluation of the data was performed by using software SPSS version 22. The mean Hb value was 11.650 ± 1.4 g / dL. Anemia was determined in 34 pregnant women (48.6%). In periodic follow up visits the mean weight of babies was 3325 g in the group without anemia and 2950 g in the group with anemia, the difference was statistically significant (p<0.001). our study showed the birth weight of the infants was found to be lower in the anemic group. For this reason, complete blood count should be performed and if necessary, iron supplement should be initiated during routine controls of mothers during pregnancy.Öğe Neural precursor cell-expressed developmentally down-regulated 4-like: a new biomarker in the pathophysiology of endometrial cancer(SAGE PUBLICATIONS LTD, 1 OLIVERS YARD, 55 CITY ROAD, LONDON EC1Y 1SP, ENGLAND, 2018) Yılmaz, Ercan; Gül, Mehmet; Melekoğlu, Rauf; Coşkun, Ebru İnci; Gül, Semir; Baştemur, Ayşe GülçinObjectives Endometrial cancer is the most frequent tumor of the female genital tract. Ubiquitin is a small protein (8.5 kDa) found in all eukaryotic cells, binds to substrate proteins via a three-phase enzymatic pathway referred to as ubiquitination and plays an important role in cellular stability. Neural precursor cell-expressed developmentally down-regulated 4-like (NEDD4L) functions in the last phase of this enzymatic process. In this study, we investigated NEDD4L protein expression in endometrial cancer. Methods The study participants were divided into patients with benign endometrial pathologies (Group 1, n=23), patients with endometrial hyperplasia (Group 2, n=21) and patients with endometrial cancer (Group 3, n=20). NEDD4L expression was detected by immunohistochemical staining and H scores were calculated to standardize staining intensity. Statistical analysis was performed using SPSS 16.0. Results NEDD4L expression levels according to H scores were significantly lower in patients diagnosed with endometrial cancer compared with those with benign endometrial pathologies. Although pregnancies with concomitant malignancies have been reported, urological tumours are possibly the most rarely identified tumours during pregnancy. Renal cell carcinoma appears to be the most common urological malignancy during pregnancy. In this case report, we discuss successful management of a patient who was diagnosed with renal cell carcinoma during the antenatal period.Öğe Ruptured tubal pregnancy with very low βhCG levels: a case report(Turgut Özal Tıp Merkezi Dergisi, 2016) Düz, Senem Arda; Eraslan ,Sevil; Coşkun, Ebru İnciAbstract Ectopic pregnancy is a pregnancy that occurs outside the uterine cavity. Although tubal ectopic pregnancy is more common, it can be rarely seen in abdominal cavity, ovarium and cervix. Mortality caused by rupture of the ectopic pregnancy decreased dramatically with the introduction of sonography and measurement of serum β-hCG (beta-human chorionic gonadotropin) levels. However there is no correlation between the clinical findings and β-hCG levels. A 29-year-old nullipar woman with amenorrhea had a β-hCG level of 1051 mUI/mL and ultrasonographic findings were pointing to tubal pregnancy. Because of the patient’s stable clinical condition, it was decided to follow the patient with expectant management. When she was admitted with severe abdominal pain, the β-hCG level was 39,8 mIU/ml and ultrasonography showed a large amount of fluid in the abdominal cavity. The patient underwent laparotomy (right) and salpengectomy. Even though β-hCG levels are low and declining, the possibility of rupture should be kept in mind. Keywords: Ruptured Ectopic Pregnancy; Tubal Pregnancy; Low β-hCG Levels.Öğe Treatment of unruptured cornual pregnancies by local injections of methotrexate or potassium chloride under transvaginal ultrasonographic guidance(Professıonal medıcal publıcatıons, panorama centre, rm 522, 5th floor, bldg 2, raja ghazanfar alı rd, po box 8766, saddar, karachı 00000, pakıstan, 2018) Tuncay, Görkem; Karaer, Abdullah; Coşkun, Ebru İnci; Melekoğlu, RaufObjective: To demonstrate the outcome of intralesional management and show the safety of local treatment of cornual pregnancy. Methods: Eight patients were treated with local methotrexate or potassium chloride injection. All patients underwent transvaginal ultrasound examination and were diagnosed by the criteria defined by TimorTritsch. In the case of fetal heart beat observation, potassium chloride was injected; and in the case of no heart beat detection, methotrexate was used. A follicle aspiration needle was inserted directly into the gestational sac under transvaginal guidance. Results: Although it has been considered to be a risk factor, none of the patients in our study had previous ectopic pregnancy, history of infertility / in vitro fertilization, or cornual pregnancy. One of the patients had a medical history of abortion. In four cases, methotrexate was injected, and three patients received potassium chloride as a local treatment. None of the patients had any complication in the peri- or postoperative period. Conclusion: Using a local approach, the treatment agent can reach the area of the cornual pregnancy in high concentrations. Based on this case series, a local approach seems to be an effective and fertilitysparing method for treating unruptured cornual pregnancies.
