    Clinical Features of Children With Henoch-Schonlein Purpura Risk Factors Associated With Renal Involvement

    Introduction. This study aimed to evaluate renal involvement and factors affecting the prognosis in patients with Henoch-Schonlein purpura (HSP). Materials and Methods. The outcomes of 107 children diagnosed with HSP who had been followed up for at least 6 months were reviewed. Results. Renal involvement was observed in 26.1% of the patients. The mean age of the patients with renal involvement was 8.8 +/- 4.0 years as compared to 7.1 +/- 2.9 years in the patients without renal involvement (P = .02). The risk of renal involvement was found to be significantly higher in the patients who were 10 years old and over (P < .001). In the group with renal involvement, the frequency of scrotal involvement was significantly higher than that of the group without renal involvement (P = .02). The mean serum immunoglobulin A level of the patients with renal involvements was significantly higher (P = .04) and the mean serum complement C3 levels was significantly lower (P = .04) than those of the patients without renal involvement. None of the patients with renal involvement reached end-stage kidney failure. No significant relationship was observed between the development of renal involvement and early steroid treatment. Conclusions. This study proposes that in old children with HSP, elevated serum immunoglobulin A levels, decreased serum complement C3 levels, and scrotal involvement are associated with renal involvement. We failed to find any effect of steroid treatment on development of renal involvement.

    Comparison of brain apparent diffusion coefficient value in naturally and assisted conceived newborns

    Background: Our aim was to assess the value of brain apparent diffusion coefficient (ADC) in Intra Cytoplasmic Sperm Injection (ICSI) and spontaneously conceived preterm singletons. Methods: Twenty ICSI-conceived preterm singletons and 20 gestational age matched spontaneously conceived preterm singletons were studied. All subjects underwent diffusion-weighted imaging (DWI). The main outcome measure was the brain ADC value in the in ICSI-conceived and naturally conceived newborns. Results: Children born after ICSI had an increased ADC value than the controls. The ADC values of ICSI-conceived singletons were higher than those of spontaneously conceived singletons at cerebellum, corpus striatum, frontal lobe, occipital lobe, and temporal lobe white matter. The mean Apgar score, birth weight, height, and head circumference of ICSI children were found to be similar to naturally conceived children. Conclusions: Measuring ADC value may be a promising marker in identifying neurological outcome of ICSI-conceived newborns.

    Evaluation of the Attitudes of Pediatric Residents and Medical Students Towards Child and Death

    Introduction: The aim of this study was to determine the experiences and attitudes of pediatric residents and medical students about approaching to child who encounters with death and to the family who have lost their child. Materials and Methods: Totally 50 pediatric residents and medical students from the Department of Pediatrics at Inonui University included in this study. A 5-part evaluation form that was prepared by the authors was applied. These parts consist of questions about frequency of experiences with child death, competency levels related to sharing bad news and explaining death to children and family. Results: The mean age of the participants was 26.9 +/- 4.0 years and 56% of them were males. The percentage of experiencing child death was 92%. Of the respondents 80% reported that they didn't feel adequately trained about sharing bad news. 68% of the residents and 76% of the students stated that they haven't been trained on approaches on death and on fatal disease and all of the participants agreed on necessity of taking education. Of the respondents 46% consider that children begin to perceive death issue at ages of 6-7. Conclusions: In this study, it was detected that with the increase of frequency of experiencing child death, the rate of competency level wasn't rising as well as having difficulty with sharing bad news wasn't decreasing. There are misconceptions in communities about how to explain death concept to child and how to console families who have lost their child. The results of this study showed that experiencing death frequently alone isn't effective in increasing the competency level on sharing bad news but education constitutes a significant part. Physicians should learn the developmental characteristics of death during medical education and should have necessary knowledge and skills to manage the process after loss.

    Evaluation of the knowledge, attitudes and concerns of the mothers of children who applied to the hospital during the COVID-19 pandemic

    Aim: During the coronavirus disease 2019 (COVID-19) pandemic many families have faced difficulties such as using masks, maintaining social distancing, and isolating themselves to stop spreading the virüs. However, there is limited data about how mothers of affected children with acute/chronic diseases overcome this challenging and stressful process. Methods: Between January 2021 and June 2021, mothers of 315 children aged 1-17 years admitted to the Pediatrics Outpatient Clinic of Inonu University were included in our study. Sociodemographic information, attitudes, and knowledge levels about the prevention of the disease towards COVID-19 were reviewed and the Stait-Trait Anxiety Inventory test was applied. Results: The overall mean score of knowledge of the mothers for COVID-19 disease was 4.9 ± 1.4 (max=8). The low educational level and employment status of the father, the presence of any chronic disease in the family, and the presence of a chronic disease in their children increased the anxiety scores in those mothers. Conclusion: Since COVID-19 still affect the community, it is very important to understand its psychological consequences on mothers who have children or close relatives with chronic diseases and to develop policies to improve their emotional and mental status.

    Human milk banking and milk kinship: Perspectives of mothers in a Muslim country

    Aim: The present study aims to determine the knowledge, attitudes and views of mothers regarding infant feeding, breast milk, wet-nursing, milk kinship and human milk banks. Methods: This descriptive cross-sectional study was composed of 1042 mothers who delivered at two different hospitals in Turkey. Results: Almost half of the participating mothers, 49.9%, agreed with the establishment of alternative HMBs in Turkey. Only 7.7% of the mothers in this study expressed views in favour of the establishment of Western-style HMBs. Approximately half of the mothers (42.4%) indicated that they were against the establishment of any kind of HMBs in Turkey. Only 9.2% of the mothers in this study stated that they would volunteer to donate their breast milk to the Western-style HMBs, and only 6.9% of the mothers approved obtaining milk from this type of HMB. Finally, 44.2% of the mothers stated that they would donate their breast milk to the alternative HMBs, and 31.9% of the mothers approved obtaining milk from this type of HMB. Conclusion: This is the first study conducted among mothers in a Muslim community about issues such as infant feeding, breast milk, wet-nursing, milk kinship and HMBs. The majority of the mothers in this study are against the establishment of Western-style HMBs, whereas they have a more positive response to an alternative HMB when their religious concerns are relieved.

    Sebaceous hyperplasia is a common benign proliferation of the sebaceous gland. It commonly presents in middle-aged people as soft, yellow papules with central umbilication on the face, particularly on the forehead. We report a newborn with striking unilateral sebaceous hyperplasia and suggest that this may represent a unique sebaceous hamartoma rather than premature sebaceous hyperplasia.

Bu sendrom bulunan çocukların çoğu şiddetli solunumsal yetmezlik gösterir ve erken süt çocukluğu döneminde ciddi ventilatör desteğine ihtiyaç duyabilir. Sendromun renal displazi, retinitis pigmentosa, hepatik ve pankreatik fibrozu içerebilen geniş bir klinik spektrumu mevcuttur. Doğum öncesi akondroplazi tanısı bulunan ve doğum sonrası ciddi solunumsal yetmezlik gösteren ve Jeune sendromu tanısı konulan bir hastayı sunduk.

    Neurodevelopmental evaluation of newborns who underwent hypothermia with a diagnosis of hypoxic ischemic encephalopathy based on the Bayley-III scale

    Background/ aim: Hypoxic ischemic encephalopathy (HIE) is one of the common causes of mortality and morbidity in newborns. Despite therapeutic hypothermia, an important treatment with proven efficacy, the morbidity and mortality rates remain high. The aim of this study was to neurodevelopmentally evaluate patients who underwent therapeutic hypothermia.Material and method: Included herein were patients who underwent hypothermia between 2018 and 2020. Their medical files were reviewed retrospectively, and their demographic and clinical information was recorded. Patients whose contact information was available were called to the developmental pediatrics outpatient clinic for a neurodevelopmental evaluation. The Bayley Scales of Infant and Toddler Development 3rd Edition (Bayley-III) was used as the evaluation tool. Laboratory values and clinical parameters of the patients were further analyzed.Results: It was found that 42 patients underwent hypothermia in 3 years, of whom 14 (33.3%) had died. Of the 28 patients who were discharged, 20 children could be reached, and a neurodevelopmental evaluation was performed. Developmental delay in the cognitive area was detected in 11 (55%) patients, delay in the language area was found in 9 (45%) patients, and delay in the motor area was found in 11 (55%) patients. The correlation and regression analysis results determined that the time to start cooling was the most effective common factor in all 3 fields of scoring. Conclusion: The time to start cooling is related to the neurodevelopmental outcomes of patients with HIE. The earlier cooling is started, the better the neurodevelopmental results. Despite therapeutic hypothermia, the neurodevelopmental development of infants may be adversely affected. These patients should be followed-up neurodevelopmentally for a long time.

    Parenting knowledge of child development in Turkish mothers and fathers

    Objective: Many studies have shown a positive relationship between parenting knowledge and early childhood development. We aimed to assess parenting knowledge about early childhood development and child-rearing in a sample of Turkish mothers and fathers. Methods: A total of 106 married Turkish couples with children under the age of three participated in our study. Mothers and fathers completed 45 questions of the Knowledge of Infant Development Inventory (KIDI) and a sociodemographic questionnaire, individually. The KIDI is widely used in the international literature on child development and parenting knowledge. Permission to use the inventory was obtained from its author. Results: The median KIDI questionnaire scores were 8 (min -max: (-10)- 20) for mothers and 6 (min -max: (-14)- 20) for fathers (maximum score 30). The average knowledge score obtained by mothers was significantly higher than the average score obtained by fathers (p=0.024). Maternal education is associated with parenting knowledge (p<0.001). Mothers had the most experience in changing diapers and dressing their children, while fathers had the most experience in playing with their children. The least experience for mothers and fathers was reading books with their children. It was found that mothers learned the information about their children mostly by talking to their families and fathers learned mostly by talking to their wives. Conclusion: The results of the study show that Turkish couples' parenting knowledge, especially in child development, is an area that needs to be reinforced. Education level played an important role in parenting. Policies to improve parental education may also include increasing parental knowledge of child development.

    Referral Trends in Two Pioneering Developmental-Behavioral Pediatric Centers in Turkey

    Objective: To inform professionals pioneering developmental-behavioral pediatrics (DBP) services in low- and middle-income countries (LMICs), we aimed to examine referral trends in 2 pioneering DBP centers at different locations in Turkey and to ascertain whether the official establishment of DBP as a subspecialty affected these trends. Method: This longitudinal observational study included all children referred to Ankara (AUDPD) and Inonu (IUDPD) Universities' Developmental Pediatrics Divisions between 2010 and 2018. We examined the sources of referrals and the independent effects of time and the establishment of DBP as a subspecialty on referral volume using Negative Binomial Regression (NBR) models. Results: Of 8,051 children, most were boys (58%) and under 24 months of age (72%). Most referrals were from the pediatric and pediatric surgery department clinics (85%); less than 1% were from child and adolescent psychiatry, and none were from family physicians. The NBR models showed that yearly, the referral volume increased significantly, 1.18-fold (95% confidence interval [CI] = 1.09-1.28) and 1.48-fold (95% CI = 1.20-1.82) for AUDPD and IUDPD, respectively. Compared with the trend of referrals before, the trend after the establishment of DBP as a subspecialty increased significantly at AUDPD, but not IUDPD. Conclusion: The increase in referrals to DBP over time is encouraging to professionals working to advance DBP services and training in LMICs even if DBP is not officially established as a subspecialty. Additional efforts may be needed to improve recognition and use of DBP services by community physicians and allied disciplines that provide services to children.

    Sleep patterns of young children with newly diagnosed atopic dermatitis

    Introduction: Even though atopic dermatitis (AD) most often begins in the first year of life, it is not well known whether sleep disturbances occur following the onset of the disease or whether they develop later. Aim: To determine the sleep patterns of young children already diagnosed with AD in comparison to a control group by using a validated questionnaire, the Brief Infant Sleep Questionnaire (BISQ). Material and methods: Forty-six children with a new diagnosis of AD and 60 healthy children, aged 3-36 months, participated in the study. Their sleep behaviors were assessed using the BISQ along with a structured sociodemographic data form. Results: It was found that when compared with healthy children, children with AD did not have decreased daily total sleep duration (p = 0.1); however, it was found that they woke up more frequently at night (52.2% vs. 40%, p = 0.4) and they stayed awake significantly longer than 60 min when they woke up (41.3% vs. 11.7%, p < 0.05). In addition, mothers of children with AD reported that their children had three times as many sleep problems compared to the reports of mothers of healthy children. Conclusions: The findings of this study showed that sleep disturbance was more common in young children with already diagnosed AD, and the BISQ provided a practical way to assess the sleep patterns. The use of a screening tool to enable early identification and treatment of childhood sleep problems among patients with AD should be encouraged.
