Sol Kalp Yetmezliği Olan Geriatrik Yaş Grubundaki Hastaların MNA, CONUT ve SGA ile Malnütrisyon Durumlarının Değerlendirilmesi Giriş ve Amaç: Malnütrisyon; yetersiz ya da dengesiz beslenme ile karakterize olan, besinlerin uygun alımında ya da sindiriminde olan problemler sonucu ortaya çıkabilen, geriatrik yaş grubu hastalarda mortalite ve morbiditede anlamlı artışlara neden olan ciddi bir halk sağlığı problemidir. Sol kalp yetmezliği olup ejeksiyon fraksiyonu (EF) ? 40 olan 65 yaş ve üstündeki hastaları Mini Nütrisyonel Test (MNA), Mini nütrisyonel test kısa form (MNA-SF), Kontrollü Nütrisyonel Değerlendirme (CONUT) ve Subjektif Global Değerlendirme (SGA) testleriyle malnütrisyon açısından değerlendirdik. Materyal ve Metod: İnönü Üniversitesi İç Hastalıkları ve Kardiyoloji kliniklerinde yaptığımız bu çalışma ile MNA, MNA-SF, SGA ve CONUT testleriyle KY hastalarında malnütrisyon durumunu göstermeyi amaçladık. Hastalarımızı 65 yaş üstünde ve sol kalp yetmezliği olup EF'si ?%40 olan gruptan aldık. Çalışmamızda 95 erkek ve 43 kadın olmak üzere toplam 138 hasta yer aldı. Anket yaptığımız hastaların aynı zamanda antropometrik ölçümlerini (üst kol ve baldır çevresi) yaptık. Çalışmamızda; spironolakton, furosemid, beta bloker (BB), ACE inhibitörü / ARB kullanımlarını sorguladığımız hastaların aynı zamanda, hemoglobin, serum kreatinin, serum elektrolit, proBNP (Beyin Natriüretik Peptit), kan üre azotu (BUN), kolesterol, albümin ve pulmoner arteryel basınç (PAB) değerlerini verilerimize ekledik. Bulgular: Hastaların yaş ortalaması 73,6 ± 7,6 idi. MNA ile hastaların %16,7'sinde (n:23) malnütrisyon bulunurken bu oran SGA da %17,4 (n:24), CONUT skorunda ise %13 (n:18) olarak bulundu. VKİ, albümin, kolesterol değerleriyle, MNA ve CONUT skorları aralarında istatiksel olarak anlamlı ilişki görüldü. CONUT ile MNA arasında malnütrisyonu göstermede istatiksel olarak anlamlı ilişki görüldü (r: -0,331, p<0,01). CONUT ile MNA-SF arasında istatiksel olarak anlamlı ilişki görüldü (p<0,01, r: -0,284). SGA-C ile MNA arasında istatiksel olarak anlamlı ilişki görüldü (p<0,01). Sonuç: Geriatrik yaş grubundaki kalp yetmezliği hastalarında; MNA, MNA-SF ve SGA malnütrisyonu göstermede ve taramada etkin olup güvenle uygulanabilir. Evaluation of Malnutrition Status with MNA, CONUT and, SGA in Patients with Left Heart Failure in Geriatric Age Group Introduction and Purpose: Malnutrition; Inadequate or unbalanced nutrition, which is caused by problems in the proper intake or digestion of nutrients, is a serious public health problem that causes significant increases in morbidity and mortality in geriatric age group. Patients with left heart failure who were 65 years old and older with an ejection fraction (EF) ? 40 were included in our study. Evaluated in terms of malnutrition with Mini Nutritional Assessment (MNA), Mini Nutritional Assessment Short Form (MNA-SF), Controlled Nutritional Status (CONUT) and Subjective Global Assessment (SGA) tests. Material and method: In the present study we conducted in İnönü University Internal Medicine and Cardiology Clinics, we aimed to show malnutrition status in patients with HF, MNA, MNA-SF, SGA and CONUT tests. İn the present study, we included patients 65 years and older with ejection fraction ? 40%. In the present study, 138 patients, 95 men and 43 women were included. We also conducted anthropometric measurements (upper arm and calf circumference) of the patients in which we conducted the questionnaires. In the present study, we added the values of hemoglobin, serum creatinine, serum electrolytes, proBNP, blood urea nitrogen (BUN), cholesterol, albumin and pulmonary arterial pressure (PAB) to the data of patients in whom we questioned the use of spironolactone, furosemide, beta blocker (BB), ACE inhibitor / ARB. Results: The mean age of the patients was 73.6 ± 7.6 years. In the present study, 16.7% (n:23) of patients with MNA had malnutrition and 17.4% (n:24) for SGA and 13% (n:18) for CONUT score. There was a statistically significant relationship between BMI, albumin, cholesterol levels with MNA and CONUT scores. There was a statistically significant relationship between CONUT and MNA in demonstrating malnutrition (r: -0,331, p <0,01). There was a statistically significant relationship between CONUT and MNA-SF (p<0,01, r:-0,284). There was a statistically significant relationship SGA-C and MNA in demonstrating malnutrition (p<0,01). Conclusion: In patients with heart failure in geriatric age group, MNA, MNA-SF, and SGA tests are effective in diagnosis and screening of malnutrition and can be safely administered.
