Özet
Amaç: Bu çalışmanın amacı, KBF ve KF'nin palatal bölgedeki yara iyileşmesi, palatal doku kalınlığı ve postoperatif hasta konforu üzerine etkilerinin değerlendirilmesidir. Materyal ve Metot: Standart boyutlarda otojen yumuşak doku grefti alımı planlanan 38 hasta KBF (n=12), KF (n=13) ve kontrol (n=13) olmak üzere üç gruba ayrılmıştır. Palatal doku kalınlığı başlangıç, 1. ve 3. aylarda ölçülmüş; yara iyileşmesi H?O? testi, MMS ve LTH indeksiyle, ağrı ve postoperatif konfor ise analjezik kullanımı, VAS ve OHIP-14 ile 7., 14. gün ve 1. aydaki kontrollerde değerlendirilmiştir. Bulgular: KBF ve KF gruplarında, başlangıca kıyasla palatal doku kalınlığında anlamlı artış gözlenmiştir. 3. ay sonunda en yüksek artış KBF grubunda saptanmış ve bu değer kontrol grubuna kıyasla istatistiksel olarak anlamlı bulunmuştur (p=0.011). 0–7. günlerde KBF ve KF gruplarında VAS skorları daha düşük, ayrıca 4–7 günlerde KBF grubunda analjezik ihtiyacı kontrol grubuna göre az bulunmuştur (p<0.05). Gruplar arasında H?O? testi açısından anlamlı fark bulunmamıştır (p>0.05). MSS değerlendirmelerinde KBF grubunda renk, KBF ve KF gruplarında kontür daha hızlı normalleşmiştir (p<0.05). LTH indeksine göre en erken iyileşme KBF grubunda başlamış, KBF ve KF gruplarında kontrol grubuna kıyasla daha erken tamamlanmıştır (p<0.05). OHIP-14 skorları tüm gruplarda postoperatif 14. günde anlamlı iyileşme göstermiş, fonksiyonel kısıtlılık KF grubunda 14. günde tamamen ortadan kalkmıştır (p=0.016). Sonuç: KBF ve KF yara iyileşmesini hızlandırarak ve postoperatif morbiditeyi azaltarak iyileşme sürecinde olumlu etkilere sahiptir. Objective: The aim of this study was to evaluate the effects of CGF and CF on wound healing, palatal tissue thickness, and postoperative patient comfort at the palatal donor site. Materials and Methods: 38 patients scheduled for autogenous soft tissue graft harvesting were randomly divided into three groups: CGF (n=12), CF (n=13) and control (n=13). Palatal thickness was measured at baseline, 1 and 3 months. Wound healing was assessed on days 7 and 14, and at 1 month using the H?O? test, MMS scale, and LTH index. Pain and comfort were evaluated by analgesic intake, VAS and OHIP-14. Results: Both CGF and CF groups showed significant increases in palatal thickness compared to baseline. At 3 months, the greatest increase was in the CGF group, significantly higher than control (p=0.011). During the first postoperative week, VAS scores were lower in both test groups and analgesic intake decreased in the CGF group between days 4–7 (p<0.05). No significant differences were found in the H?O? test (p>0.05). MMS evaluations revealed earlier color normalization in CGF, while contour normalized faster in both CGF and CF (p<0.05). According to the LTH index, healing began earliest in CGF, with both test groups completing healing earlier than control (p<0.05). OHIP-14 scores improved significantly in all groups by day 14, with functional limitation resolving only in CF (p=0.016). Conclusion: CGF and CF positively influenced the healing process by accelerating wound healing and reducing postoperative morbidity.
