Özet
Amaç: Bu araştırma karaciğer nakli donörlerine ameliyat sırasında dinletilen müziğin bazı hemodinamik değerler ve kortizol düzeyine etkisini belirlemek amacıyla yapıldı. Materyal ve Metot: Araştırma, randomize kontrollü olarak Aralık 2020-Kasım 2022 tarihleri arasında, İnönü Üniversitesi Karaciğer Nakli Enstitüsü ameliyathanelerinde yürütüldü. Örneklemi 30 müzik, 30 sessizlik ve 30 kontrol grubu olmak üzere 90 canlı karaciğer donörü oluşturdu. Veriler Hasta Tanıtım, Hemodinamik Parametre Kayıt ve Kortizol Düzeyi Kayıt Formları kullanılarak toplandı. Müzik grubundaki hastaların hemodinamik parametreleri ve kortizol düzeyleri belirlendikten sonra 30 dakikalık müzik uygulaması yapıldı. Uygulamadan hemen sonra (30. dk) tekrar hemodinamik parametreleri ve kortizol düzeyleri saptandı. Verilerin analizinde; sayı, yüzdelik, ortalama, standart sapma, ki-kare, tekrarlı ölçümlerde iki yönlü varyans analizi testleri kullanıldı. Bulgular: Müzik, sessizlik ve kontrol grupları arası karşılaştırmada; ön test ve son test nabız hızı, sistolik kan basıncı, diastolik kan basıncı, solunum hızı ve oksijen satürasyonu puan ortalamaları arasında anlamlı bir farklılık olmadığı belirlendi (p>0.05). Müzik grubundaki hastaların son test serum kortizol düzeyinin sessizlik ve kontrol grubuna göre daha düşük olduğu belirlendi (p<0.05). Grup içi karşılaştırmalarda ise; müzik grubunun nabız hızı, sistolik ve diastolik kan basıncı, oksijen satürasyonu ve kortizol düzeyi arasındaki fark anlamlı bulundu (p<0.05). Kortizol düzeyinin ön test ve son test değerleri arasındaki farklar incelendiğinde puan ortalamalarındaki azalmanın müzik grubunda daha fazla olduğu belirlendi (p<0,001). Sonuç: Karaciğer nakli donörlerine ameliyat sırasında dinletilen müziğin; nabız hızı, sistolik ve diastolik kan basıncını düşürdüğü, oksijen satürasyonunu arttırdığı, solunum hızı üzerinde herhangi bir etkisinin olmadığı, kortizol seviyesini düşürdüğü belirlendi. Aim: This study was conducted to determine the effect of music played to liver transplant donors during surgery on some hemodynamic values and cortisol levels. Material and Method: The randomized controlled study was conducted between December 2020 and November 2022 in the operating rooms of İnonu University Liver Transplant Institute. The sample consisted of 90 living liver donors, including 30 music, 30 silence and 30 control groups. Data were collected using the Patient Description, Hemodynamic Parameter Record and Cortisol Level Record Forms. After determining the hemodynamic parameters and cortisol levels of the patients in the music group, 30 minutes of music was applied. Hemodynamic parameters and cortisol levels were determined again immediately after the application (30 minutes). In the analysis of data, two-way analysis of variance tests were used for number, percentile, mean, standard deviation, chi-square, and repeated measurements. Findings: In the comparison between the music, silence and control groups, it was determined that there was no significant difference between the pre-test and post-test pulse rate, systolic blood pressure, diastolic blood pressure, respiratory rate and oxygen saturation mean scores (p>0.05). In intragroup comparisons, the difference between pulse rate, systolic and diastolic blood pressure, oxygen saturation and cortisol level of the music group was found to be significant (p<0.05) When the differences between the pre-test and post-test values of cortisol levels were examined, it was determined that the decrease in the mean score was higher in the music group (p<0.001). Conclusion: It was determined that music played to liver transplant donors during surgery decreased pulse rate, systolic and diastolic blood pressure, increased oxygen saturation, had no effect on respiratory rate, and decreased cortisol levels.
