Özet
Öz: Tilberkillozlu hastalarda akuffaz reaktanlarinin, tedaviye cevabi ve hasiahgin aktivitesini belirlemede muhte- mel marker olabilecegi one silrulmektedir. Aktif akciger tiiberkulozu olan 30 hastada akutfaz reaktanlan uze¬ rine tuberkiiloz tedavisinin etkisini degerlendirmek if in, tedaviden Once ve tedavinin 6. aymda serum trans- ferrin, albumin ve kompleman C3 ve C4 gibi akutfaz reaktanlarinin seviyelerini ve eritrosit sedimantasyon, hi- zini (ESH) b'lgtuk. Turn akutfaz reaktanlarinin serum seviyeleri normal sinirlar icinde olasina ragmen, teda- vi sirasmda duzeylerinde degisiklikler meydana geldi. Transferrin (190,6 ± 55,9 dan 222,8 ± 76,3 mg/dL'ye) (p<0.007) ve albumin (4,0 ± 0,7dan 4,7± 0,4g/dL'ye) (p<0.0001), seviyeleri tuberkiiloz tedavisiyle artarken, kompleman C3 (173,7 ±5,8 dan 121,2 ± 31,5 mg/dL'ye) diizeyinde azalma goriildu (p<0.0001). Kompleman C4 diizeyinde onemli degisiklik gdriilmedi. ESH baslangigta yilksek (50 ± 20 mm/saat) olarak olguldiX ve teda- viyle normale (10 ± 3 mm/saat) dondii (p<0.0001). Sonugta ESHyamnda transferrin, albumin ve kompleman C3 dilzeylerinin tuberkulozda tedavi cevabinin izlenmesinde kullanish bir marker olarak degerlendirilebilece- gi dusilnilldu.
Abstract: Several acute phase reactant levels have been proposed as possible markers to monitor disease activity and res¬ponse to the treatment in patients with tuberculosis. In order to evaluate the effect of anti-tuberculous che¬motherapy on acute phase reactants, we measured the serum levels such of acute phase reactants as transfer¬rin, albumin, complement C3 and C4 and erytrocyte sedimentation rate (ESR) in 30 patients with active pul¬monary tuberculosis before and after anti-tuberculous chemotherapy. Although all acute phase reactant levels were within normal limits, before and after the treatment their levels changed during treatment. While there we¬re an increase in the concentrations of transferrin (190,6± 55,9 versus 222,8 ± 76,3 mg/dl) (p<0.007) and al¬bumin (4,0 ± 0,7 versus 4,7 ± 0,4 g/dL) (p<0.0001), there was a significant decrease in complement C3 level (173,7 ± 5,8 versus. 121,2 ± 31,5 mg/dL) (p<0.0001) in patients with pulmonary tuberculosis. The ESR before treatment significantly increased (50 ± 20 mm/h) and returned to the normal levels (10 ± 3 mm/h) after 6 months of anti-tuberculous treatment (p< 0.0001). No significant decrease was observed in the level ofcomplement C4. In conclusion, transferrin, complement C3 and albumin are considered to be the biochemical mar-&#9632; kers capable of being used to predict and monitor the clinical response to the chemotherapy
