Özet
Öz: Amaç: 50 gr glukoz yükleme testi yüksek, 100 gr oral glukoz tolerans testi normal olan gebelerde glukozintoleransını araştırmak. Yöntem: İnönü Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı’na başvuran gebeler, gebeliğin 24-28. haftaları arasında yapılan 50 gr glukoz yükleme ve100gr. oral glukoztolerans testi sonucu, Tip-2 diabetes mellitus tanısı ve yenidoğan ağırlığı kriter alınarak 5 çalışma grubu oluşturuldu. Grup 1 (n: 6) gestasyonel diabetes mellitus, Grup 2 (n: 8) Tip-2 diabetes mellitus, Grup 3 (n: 10) 50 gr glukozyükleme testi yüksek, 100 gr oral glukoz tolerans testi normal, Grup 4 (n: 8) kontrol grubu (50 gr glukoz yükleme testi normal ve yenidoğanları gebelik haftasına göre normal ağırlıkta olan gebeler) ve Grup 5 (n: 8) 50 gr glukoz yükleme testi normal makrozomik yenidoğanı olan gebelerden oluştu. Çalışmaya alınan grupların maternal ve kordon kan örneklerinde glukoz intoleransını araştırmak amacı ile IGF-I, II ve IGFBP 1,2,3 düzeyleri ölçüldü. IGF I, II ve IGFBP-1,3 ölçümleri IRMA (Immunoradiometric Assay), IGFBP-2 ölçümleri ise RİA (Radioimmunassay) yöntemleri kullanılarak yapıldı. Sonuçların değerlendirilmesinde One-way ANNOVA karşılaştırma ve Spearman korelasyon testleri kullanıldı. Sonuç: IGF-II ve IGFBP-1,3 düzeyleri 4 grupta da kontrol grubuna göre istatistiksel anlamlılıkta yüksek bulundu. (p<0.000, p<0.000, p<0.05). IGF-I ve IGFBP-2 düzeyi makrozomik yenidoğanı olan grupta diğer gruplara göre istatistiki anlamlılıkta yüksekti (p<0.001).
Abstract: Objective: The aim of this study is to evaluate the carbonhydrate metabolism in pregnants with abnormal glucose challenge, but normal 100 g-3-hour oral glucose tolerance test. Methods: Pregnants who were delivered at &#8249;nönü University School of Medicine Department of Obstetrics and Gynecology and meet the criteria included the study.Group 1 (n: 6) gestational diabetes mellitus, Group 2 (n: 8) Type 1 diabetes mellitus, Group 3 (n: 10) serum glucose levels were 140mg/dl £ after glucose challenge test,but normal 100 g-oral glucose tolerance test, Group 4 (n: 8) pregnants with normal glucose challenge test and have appropriate-for-gestational-age neonate as control group,Group 5(n: 8) pregnants with normal glucose challenge test and have macrosomic neonate were included the study.Maternal blood was collected to evaluate the carbonhydrate metabolism by comparing the IGF-I,II and IGFBP-1,2,3 levels. IGF I-II and IGFBP1,2,3 measurements were done by RIA and IRMA technics respectively. One-way ANOVA test was used for group comparison and Spearman correlation test was used for correlation between groups. P<0.05 was required for statistical significance. Results: IGF-1, IGF-II and IGFBP-1,3 levels were high in 4 groups compared to control group and statistically significant in IGF-II and IGFBP-1,3 levels (p<0.000, p<0.000, p<0.05). IGF-I and IGFBP-2 levels were significantly high in macrosomic neonate group compared to other 4 groups (p<0.001).
