| dc.description.abstract | Bu çalışmada BT'de lumbar spirıal stenoz tanısı konuları hastalarda istirahatte ve eforu takiben nörojenik klodikasyo ortaya çıktıktan sonra kaydedilen H refleksi değişikliklerinin radikülopatiyi ortaya koymadaki değeri araştırıldı. Çalışma 21 lumbar spinal stenozlu hastada yapıldı. 21 sağlıklı birey de kontrol grubu olarak alındı. Hastaların efor öncesi ve efor sonrası H refleksi ortalama latans değerleri, latans farkı ortalama değerleri kontrol grubu değerlerine göre anlamlı derecede (sırasıyla p<0.01, p<0.002) uzamış olarak bulundu. Yine hastaların efor sonrası ve öncesi latans farkı ortalama değerleri kontrol grubu değerlerine göre anlamlı (p<0.02) derecede fazlaydı. Hasta grubunda efor sonrası-efor öncesi latans farkının 7 hastada (%33.3) 1 msn'den daha fazla olduğu saptandı ve lateral reses darlığı saptanan hastalarda, lateral reses darlığı olmayanlara göre latans farkının anlamlı derecede (p<0.03) fazla olduğu görüldü. Lateral reses darlığına osteofitik oluşumların eşlik ettiği hastalarda bu ilişki daha anlamlı idi (p<0.002). Tek taraflı semptomu olan hastalarda latans farkının bilateral semptomlu olguların latans farklarına göre (sırasıyla 1,2±1,3 msn, 0,2±0,8 msn) anlamlı derecede (p<0.05) uzamış olduğu saptandı. Sonuçlar duyusal ve motorilet değerler normal olan lumbar spinal stenozlu hastalarda proks malisinirilet değerlerin test etmekte kullanılan H refleksinde eforla anlamlı bir değişikliğin olduğunu göstermektedir. | en_US |
| dc.description.abstract | In this study, we evaluated diagnostic value of the H reflex changes in the diagnosis of radiculopathy. The patient group with lumbar spinal stenosis were diagnosed with CT. The H reflex changes were evaluated before and after effort when neurogenic claudication appeared. The patient group included 21 patients and 21 healthy subjects were evaluated as the control group. The average latency and average latency difference values of the H reflex before and after effort in patients were found to be significantly longer than the control group (respectively, p<0.01, p<0.002) Average values of latency differences before and after effort was more significant than the control group (p<0.02). In patient group, it is observed that 7 patients (33.3°%) had a before and after effort latency difference of more than 1 msec . The latency difference in patients with lateral recess stenosis was more significant (p<0.03) than the patients without lateral recess stenosis. This relationship was more significant in patients with osteophytes accompanying lateral recess stenosis (p<0.002). The latency difference in patients with unilateral symptoms (1.2 ± 1.3 msec) was significantly longer than the patients with bilateral symptoms (1.2 ± 0.8) (P<0.05).
