Spastik diplejik serebral palsili çocuklarda video tabanlı eylem gözlem eğitimi ve canlı eylem gözlem eğitiminin; motor fonksiyon, aktivite katılım ve ikincil sonuç ölçütleri üzerine etkisi

Özet
Amaç: Bu çalışmanın amacı spastik diplejik Serebral Palsi'li çocuklarda Video Tabanlı Eylem Gözlem Eğitimi ve Canlı Eylem Gözlem Eğitiminin; motor fonksiyon, aktivite katılım ve ikincil sonuç ölçütleri üzerine etkilerini incelemektir. Materyal ve Metot: Çalışmaya Kaba Motor Fonksiyon Sınıflama Sistemine göre mobilite seviyeleri I, II ve III olan ve 5-14 yaş arasında 39 spastik diplejik Serebral Palsi'li çocuk dâhil edildi ve tabakalı randomizasyon yoluyla 3 gruba ayrıldı. 8 hafta boyunca 1. Grup, 20 dk konvansiyonel fizyoterapi programına ek olarak 20 dk video tabanlı eylem gözlem eğitimi; 2. Grup, 20 dk konvansiyonel fizyoterapi programına ek 20 dk canlı eylem gözlem eğitimi; 3. Grup 40 dk konvansiyonel fizyoterapi programı aldı. Katılımcılar Kaba Motor Fonksiyon Ölçütü, Çocuk ve Adölesan Katılım Anketi, Pediatrik Berg Denge Ölçeği, süreli kalk ve yürü testi, 5 kez otur kalk testi, Gillette Fonksiyonel Değerlendirme Anketi ve 1 dk yürüme testi ile değerlendirildi. Bulgular: 2. Grupta; KMFÖ-'Yatma ve Yuvarlanma' (p=0.066), 3. Grupta ise KMFÖ-'Yatma ve Yuvarlanma' (p=0.317), 'Emekleme ve Dizüstü' (p=0.063) motor alt testleri ve Gillette FDA-Yürüme ölçeği hariç (p=0.513) grupların diğer tüm değerlendirme parametrelerinde anlamlı iyileşmeler görüldü (p<0.05). Gruplar arasında KMFÖ 'Yatma ve Yuvarlanma' alt basamağı hariç (p=0.172), diğer tüm ölçüm parametrelerinde rehabilitasyon süreçlerine bağlı meydan meydana gelen artışlar açısından istatiksel olarak anlamlı farklılıklar bulundu (p<0.05) ve bu farklılık eylem gözlem eğitimleri lehineydi. Sonuç: Spastik diplejik Serebral Palsi'li çocuklarda konvansiyonel fizyoterapiye ek olarak uygulanan iki farklı eylem gözlem eğitiminin sadece konvansiyonel fizyoterapiye göre genel olarak kaba motor fonksiyon, aktivite katılım ve ikincil sonuç ölçütleri üzerine daha etkili olduğu bulundu. Aim: The aim of this study was to examine the effects of video-based action and live action observation training on motor function, activity participation and secondary outcome measures in children with spastic diplegic Cerebral Palsy. Material and Method: 39 children with spastic diplegic Cerebral Palsy between the ages of 5-14 and with mobility levels I, II, and III according to Gross Motor Function Classification System were included in the study were divided into 3 groups by stratified randomization. Group 1 for 8 weeks, 20 minutes of video-based action observation training in addition to the conventional physiotherapy program; Group 2, 20 minutes of live action observation training in addition to the conventional physiotherapy program; Group 3 received a conventional physiotherapy program for 40 minutes. Participants were evaluated with Gross Motor Function Measurement, Adolescent Scale of Participation, Pediatric Berg Balance Scale, Timed up and go test, Five times sit-tostand test, Gillette Functional Assessment Questionnaire and 1 minute walk test. Results: In the 2st group; Significant improvements were observed in all other evaluation parameters of the groups (p<0.05), except for the GMFM-'Lying and Rolling' (p=0.066), in the 3rd group, except for the GMFM-'Lying and Rolling' (p=0.317), 'Crawling and Kneeling' (p=0.063) motor dimensions and Gillette Functional Assessment Questionnaire-Walking Scale (p=0.172). Comparisons of the increases in all other measurements between the groups, except for the dimensions of GMFM-'Lying and Rolling' (p=0.172), were statistically significant (p<0.05) and this difference was in favor of action observation trainings. Conclusion: It was found that two different action observation trainings applied in addition to conventional physiotherapy in children with spastic diplegic Cerebral Palsy were more effective on gross motor function, activity participation and secondary outcome measures in general than conventional physiotherapy alone.
