KOAH akut alevlenmesinde oksidatif stres ve tedavisinin oksidan-antioksidan denge üzerine etkisi

Özet
Öz: Çalışmamız, kronik obstrüktif akciğer hastalığı (KOAH) akut alevlenme dönemindeki hastalarda, oksidatif stresi ve tedaviyle ortaya çıkan değişiklikleri incelemek amacıyla düzenlendi. Çalışmaya alınan 52 hastadan 36'sı çalışmayı tamamladı. Tüm hastalara yedi gün süreyle inhaler ipratropium bromid ve salbutamol, oral teofilin, ampisilinsulbaktam ve N-asetil sisteinin yanı sıra 2 L/dakika oksijen verildi. Tedaviden önce ve tedavinin yedinci gününde olguların arter kan gazı analizleri ve solunum fonksiyon testleri değerlendirildi. Serumda malondialdehid (MDA), glutatyon peroksidaz (GSH-Px) ve süperoksit dismutaz (SOD) düzeyleri ölçüldü. Tedavi öncesi ve sonrası sırasıyla MDA düzeyleri 2.96 ± 1.40 ?mol/L ve 1.93 ± 0.63 ?mol/L; SOD düzeyleri 32.34 ± 7.58 U/g protein ve 38.95 ± 6.47 U/g protein ve CSH-Px düzeyleri 4.69 ± 3.58 U/g protein ve 16.20 ± 6.46 U/g protein olarak bulundu. Tedavi sonrası MDA düzeylerindeki azalma ve SOD ile CSH-Px düzeylerindeki artış istatistiksel olarak anlamlıydı (p< 0.05). Çalışmamızın sonuçları, KOAH akut alevlenmesinde oksidatif stresin arttığını; oksijen tedavisi, bronkodilatör, N-asetil sistein ve antibiyotik tedavisi ile oksidatif stresin azaldığını göstermektedir.
Abstract: Our study was designed to investigate the role of oxidative stress, and the effects of treatment on oxidative stress in patients with acute exacerbations of chronic obstructive pulmonary disease (COPD). Of the 52 patients who were included in the trial, 36 completed the study. All patients were treated with ipratropium bromide inhaler, salbutamol inhaler, theophylline orally, ampicilline-sulbactam orally, N-acetylcysteine orally and oxygen with a flow rate of 2 L/minute. Arterial blood gases, pulmonary function parameters and serum levels of MDA, CSH-Px, and SOD were measured in all patients before and on the seventh day of therapy. The pre and posttreatment MDA levels were 2.96 ± 1.40 ?nol/L and 1.93 ± 0.63 ?mol/L; SOD levels were 32.34 ± 7.58 U/g protein and 38.95 + 6.47 U/g protein and GSH-Px levels were 4.69 ± 3.58 U/g protein and 16.20 ± 6.46 U/g protein, respectively. Decrement in MDA levels and increase in SOD and CSH-Px levels were statistically significant (p< 0.05). As a conclusion; oxidative stress was found to be increased in acute exacerbations of COPD and it was decreased after oxygen therapy, bronchodilator, N-acetylcysteine and antibiotic therapy.
