Özet
Çoğul Dirençli Acinetobacter baumannii Suşuna Özgü Litik Bakteriyofaj Araştırılması Amaç: Acinetobacter baumnnii özellikle yoğun bakım ünitelerinde çok sık saptanan önemli bir hastane patojenidir. Artan antimikrobiyal direnç, bu bakterinin neden olduğu enfeksiyonların tedavisini zorlaştırmış ve son zamanlarda tüm antibiyotiklere dirençli suşların yaygınlaşması sonucu önemli hasta kayıpları yaşanmıştır. Bu sorun, antibiyotik çağı öncesinde popüler olan bakteriyofaj terapisini yeniden gündeme getirmiştir. Bu çalışmada, çoğul ilaç dirençli A. baumannii suşuna özgü litik bir DNA fajının izole edilmesi, karakterizasyonu ve tedavide kullanım potansiyelinin araştırılması amaçlanmıştır. Gereç ve Yöntem: İnönü Üniversitesi atık su tesisleri ve kampüs çevresel su kaynaklarında belirlenen toplam 22 farklı bölgeden 250 su örneği alınmıştır. Bu örneklerde litik bakteriyofaj aranması çift agar yöntemi ile yapılmıştır. Potansiyel litik bakteriyofajların morfolojik ve genomik karakterizasyonları için SDS-PAGE, DNA dizi analizi ve elektron mikroskobu kullanılmıştır. Bununla birlikte, potansiyel litik faj adaylarının konak özgülüğü 130 klinik A. baumannii izolatı ve diğer gram negatif ile türler üzerinde test edildi. Bulgular: Toplanan 250 su örneğinin 12'sinde A. baumannii'ye karşı potansiyel litik etki gösteren faj bulunmuştur. Bunlardan A. baumannii'ye özgü en güçlü litik etki göstereni seçilerek ABO-Faj adı verilmiş ve karakterizasyonu gerçekleştirilmiştir. Transmisyon elektron mikroskobik incelemede bu fajın ikozahedral kapsid simetrisine sahip ve kuyruksuz bir yapıda olduğu görülmüştür. Fajın protein profili belirlenmiş ve nükleik asit dizisi veri tabanları ile karşılaştırıldığında "Acinotabacter phage WCHABP1" ile %95 homoloji gösterdiği belirlenmiştir. Sonuç: Bu çalışmada saptadığımız ABO-Fajının dirençli A. baumannii'ye karşı yüksek düzeyde etkili olduğu görülmüştür. Bakteriyofajların, böylesi enfeksiyonların tedavisi için bir tedavi alternatifi olabileceğine dair güçlü veriler elde edilmiş olup, yeni litik faj tiplerinin belirlenmesi için farklı habitatlarda kapsamlı çalışmalar yapılmasına gereksinim vardır.
Investigation of Lytic Bacteriophage Specific for Multidrug-Resistant Acinetobacter baumannii Strain Aim: Acinetobacter baumannii is an important nosocomial pathogen that is frequently detected in the intensive care units. Increasing antimicrobial resistance has complicated the treatment of the infections caused by this bacterium, and particularly in the recent years, significant patients lost have occurred as the distribution of pandrug-resistant strains. This problem has revived the bacteriophage treatment that was popular before the antibiotic era. In this study, isolation and characterization of a lytic DNA phage specific for resistant A. baumannii, and to study the therapeutic potency of this phage were aimed to be investigated. Material and Methods: A total 250 water samples were collected from the previously determined 22 points in the Inonu University wastewater treatment facility and the environmental water resources around the campus. Lytic bacteriophages were investigated in these samples with double agar method. Morphological and genomical characterization of the potential lytic bacteriophages was assessed with SDS-PAGE, DNA sequencing, and electron microscopy. Additionally, the specificity of the detected phages was alayzed on 130 clinical isolates of A. baumannii, and other gram negative bacterial species. Results: Acinetobacter baumannii-specific potential lytic phages were found in 12 of total 250 samples. The phage which showed the most potent lytic effect to A. baumannii was selected among them, named as "ABO-Phage", and further characterized. In transmission electron microscopy, it was observed that the phage was in ichosahedral capside symmetry without tale. The protein profile of the phage was determined, and when its nucleic acid sequence was compared with the database, it was shown that the phage was exhibiting 95% homology with "Acinotabacter phage WCHABP1". Conclusion It was observed that the phage found in this study, ABO-phage, was highly effective to resistant A. baumannii strains. Strong evident indicating bacteriophages can be a treatment alternative for such infections is determined, and there is need for comprehensive studies in different habitats to detect novel lytic phages types.
