Malign over tümörlerinde frozen inceleme ile tümör dokularında yapılan immünohistokimyasal analizlerin tanısal korelasyonu ve klinikopatolojik özelliklerin değerlendirilmesi

Özet
Amaç: Bu çalışmamızın amacı malign over kanserlerinde tümörün histopatolojik subtipinin frozen inceleme ile korelasyonunu belirlemek ve immunhistokimyasal analizin overyan malignitelerdeki yerini değerlendirmektir. Materyal Metot: Çalışmamız retrospektif bir çalışmadır. Çalışma evreni İnönü Üniversitesi Turgut Özal Tıp Merkezi'nde overde kitle nedeni ile opere edilmiş kadın hastalar olup, 412 hasta dosyası taranarak; hastaların operasyon özgeçmişi, laboratuvar sonuçları ve patoloji raporları retrospektif olarak değerlendirilmiştir. Patoloji sonuçları malign over kanseri gelmiş ve immünohistokimyasal olarak analiz edilmiş toplam 55 hasta çalışmaya dahil edilmiştir. Bulgular: Çalışmamızda malign over kanseri olan hastaların ortalama tanı yaşı 52,33 ±15.5; ortanca tanı yaşı 52,4 (min:14-max:82) olarak tespit edilmiştir. Hastaların pelvik kitle nedeniyle yapılan cerrahi sonrası patoloji raporları incelendiğinde 55 hastadan 34 (%61,8) hastanın endometriyal tip, 9 (%16,3) hastanın seks kord stromal tip, 3 (%5,4) hastanın germ hücreli tip, 9 (%19,3) hastanın metastatik over kanseri olduğu görülmüştür. Çalışmamızda sağkalım oranları epitelyal over kanseri için %70,6 seks kord stromal tümör için%100, germ hücreli tümör için %100 ve metastatik tümörler için %44,4 olarak bulunmuştur. Sonuçlar: Malignensi şüphesi bulunan hastaların muayene ve görüntüleme tetkiklerindeki hızlı gelişmeye rağmen tanı ve uygun tedavi planlaması için histopatoloji oldukça önemlidir. Özellikle jinekolojik onkoloji alanında uzman merkezlerde opere olan hastaların sonuçları çok daha iyi seyretmiştir. Buradaki en önemli iki faktör, tümör yükünün cerrahi olarak maksimum oranda ortadan kaldırılması için gerekli olan cerrahi tecrübe ve de iyi bir frozen patoloji incelemesidir. İmmunhistokimyasal yöntemler kanser tanı ve tedavisinde çok önemli ilerlemeler sağlamış olup, hedefe yönelik kanser tedavisinin anahtar noktası olacak niteliktedir. Aim: The aim of this study is to determine the correlation of histopathological subtype with the frozen section examination and the importance of immunhistochemical analysis for the ovarian malignencies. Material and methods: Our work is a retrospective study. The study sampling is female patients who are operated to malign mass in Turgut Özal Medical Center of İnönü University and 412 patient files are scanned; The patients' operation history, laboratory results and pathology reports were evaluated retrospectively. A total of 55 patients with pathologic malignant ovarian cancer and immunohistochemically analyzed were included in the study. Findings: The mean age of the patients with malignant ovarian cancer was 52,33 ± 15.5; the median age at diagnosis was 52.4 (min: 14-max: 82). When the pathologic reports of the patients due to pelvic mass were examined, 34 patients (61.8%) of 55 patients had endometrial type, 9 (16.3%) patients had sex cord stromal type, 3 (5.4%) patients had germ cell type, 9 19.3%) were found to have metastatic overcancer. In our study, survival rates were found to be 70.6% for epithelial overcancer, 100% for sex cord stromal tumor, 100% for germ cell tumor and 44.4% for metastatic tumors. Result: Histopathology is the most important factor for the diagnosis and appropriate treatment planning despite the rapid development of examination and screning of patients with suspected malignancy. In particular, the results of the patients who were operated and treated in the centers of which is specialized in gynecologic oncology have been much better. The two main factors are the surgical experience required to maximum surgical remove of the tumor burden and a advanced pathological examination.
