Bu çalışmada, Atatiirk Universitesi, TIP Fakültesi Araştırma
Hastanesi Nöroşirurji Kiinigi.'nde 1984-92 Ylllan arasmdaki 8
Yıllık bir süre içerisinde yatmlarak tedavi edilen alt servikal (C4
altl) spinal yaralanmasl ve ilave travmatik patolojisi olan 65 olgu
incelendi.
65 olgunun, en gend 8, en yaşlı 73 yaşında idi ve yaş ortalaması
27 idi. Ellidiirdii (%83.1)erkek, 1l'i (%16.9)kadm dnsten olan 65
hastamn, 34'ii (%52.3)dii~me, 27'si (%41.5)trafik kazasl ve 4'ii (%6.2)
ate~lisilah yaralanmasl sonucu başvurmuşlardr. Olgulardan 18'inde
(%27.7)klinik seyir iiliimle sonu9anml~tI. blen olgulann 15'inde
(%83.3)aglr niirolojik defisit, 5'inde (%27.7)agrr toraks patolojisi,
2'sinde (%11.1)intrakraniyal kitle yard!. Hastalann 6'SI (%9.2)ilk
72 saat içinde ölmüşlerdi.
Yalmz spinal yaralanmasl olan olgular da dahi yiiksek mortalite
ve morbiditeye sahip olan servikal spinal yaralanmalarda, spinal
yaralanmaya ilave olan diger sistemlere ait travmatik komplikasyonlar
bu oram daha da yiikseltrnektedir. Bu tip yaralanmalarda,
bir genel prosediiriin yanmda her hastaya iizgii bir yaklaşlm
içerisinde bulunulmasl gerektigi kanısındayız.
In this study, 65 cases with lower cervical spinal injuries assodated with multiple system trauma are analysed. The youngest patient is 8-year-old, and the oldest patient is 73-yearold, average age is 27. Fifty four of patients are male, and 11 are female. The most common cause of trauma is traffic acddent. Eighteen cases died, six of them within first 72 hours. Fifteen had severe neurological defidts, and 5 had severe thoracallesion, and also 2 cases had intracranial mass lesion. In the cervical spinal injuries with concomittant multiple system traumas, mortality and morbidity rates are higher than cervical spinal injuries alone. Therefore, we suggest that approach to general traumatic procedures should be personalized in these patients.
