Endoskopi Yardımlı Karpal Tünel Gevşetme Yapılan Hastaların Fonksiyonel Klinik Sonuçları Giriş: Karpal tünel gevşetme cerrahisinde 90'lı yıllardan itibaren endoskopi destekli karpal tünel gevşetme cerrahisi yaygınlaşmaya başlamıştır. Endoskopi destekli karpal tünel gevşetme cerrahisinde de diğer prosedürlerle benzer oranda sinir hasarı oluşma ihtimali hipotezi savunulmuştur. El bileğinde daha küçük skar izi, ağrısız ve daha hızlı iyileşme süresi olduğu hipotezi savunulmuştur. Amaç: Yaptığımız endoskopi destekli cerrahinin skorlamalarda, memnuniyet skorlarında olumlu yönde değişiklik olup olmadığını görnek, endoskopi destekli karpal tünel gevşetme cerrahisi sonuçlarına değişkenlerin( yaş, cinsiyet, meslek, cerrahi süre vb.) etkisini ortaya koyabilmek, endoskopi destekli karpal tünel gevşetme cerrahisi yapılan hastaların ameliyat sonrası durumlarını ameliyat öncesindeki durumlarıyla karşılaştırmak, endoskopi destekli karpal tünel cerrahi sonuçlarının litaretürle kıyasını yapmak, litaratürde endoskopi destekli cerrahi yapılanların açık cerrahi yapılanlara göre sayısal anlamda üstünlüklerinin çalışmamızda da olup olmadığını görebilmektir. Ayrıca kullandığımız skorlamaların birbirleriyle korele olup olmadığını görebilmek. Yöntem: Bu çalışmamızda 2012-2017 yıları arasında karpal tünel sendromu nedeniyle endoskopi destekli karpal tünel gevşetme cerrahisi yapılan hastalar taranmıştır. 25 hasta elde edilmiştir. Bu hastaların demografik özellikleri belirlenmiştir. Bu hastaların ameliyat öncesi, ameliyat sonrası skorlamaları kayda alınmıştır. Hastaların memnuniyet dereceleri kayda alınmıştır. Hastaların işe dönme süresi kaydedilmiştir. Cerrahi süreler kayda alınmıştır. Bunların ışığında skorlamaların birbiriyle korele olup olmadığı kıyaslanmış, yaşın cerrahi sonuçlara etkisinin olup olmadığı kıyaslanmış, endoskopi destekli karpal tünel gevşetme cerrahisinin sayısal sonuçları, komplikasyon oranları literatürle kıyaslanmıştır. Sonuçlar: Yaptığmız endoskopi destekli karpal tünel gevşetme cerrahisinde memnuniyet skorlarında olumlu yönde değişiklik yapmıştır. Yaptığmız endoskopi destekli karpal tünel gevşetme cerrahisini litaratürle kıyas ettiğimizde benzer sonuçlar çıkmıştır, komplikasyon oranları litaratüre göre düşük çıkmıştır. Kullandığmız skorlamaların ameliyat öncesi ve sonrası değerleri birbirleriyle korele çıkmıştır. Tartışma: Endoskopi destekli karpal tünel gevşetme cerrahisi karpal tünel sendromunda etkili bir tedavi yöntemidir. Klinik olarak sonuçları diğer yöntemlerle benzerdir. Enfeksiyon, hematom, skar hassasiyeti, skar izi gibi problemler bu yöntemde daha düşüktür. Tecrübeli ellerce yapıldığında korkulan komplikasyonlardan olan mediyan sinir hasarı riski diğer yöntemlerle benzerdir. Functional Clinical Outcomes of Patients Performed with Endoscopy Assisted Carpal Tunnel Release Introduction: Endoscopy-assisted carpal tunnel release surgery has become widespread in carpal tunnel release surgery since the 90s. The hypothesis that a similar rate of nerve damage may occur in endoscopy-assisted carpal tunnel release surgery has been advocated. It has been hypothesized that there is a smaller scar on the wrist, painless and faster recovery time. Purpose: To see whether there is a positive change in the scores and satisfaction scores of our endoscopy-assisted surgery, to reveal the effect of variables (age, gender, occupation, duration of surgery, etc.) on the results of endoscopy-assisted carpal tunnel release surgery, The aim of this study is to compare the post-operative conditions with their pre-operative conditions, to compare the results of endoscopy-assisted carpal tunnel surgery with the literature, and to see whether there are numerical superiorities of those who underwent endoscopy-assisted surgery compared to those who underwent open surgery in the literature. Also, to see if the scorings we use are correlated with each other. Method: In this study, patients who underwent endoscopy-assisted carpal tunnel release surgery for carpal tunnel syndrome between 2012 and 2017 were screened. 25 patients were obtained. The demographic characteristics of these patients were determined. Preoperative and postoperative scores of these patients were recorded. The satisfaction levels of the patients were recorded. The patients' return to work time was recorded. Surgical times were recorded. In the light of these, it was compared whether the scores were correlated with each other, whether the effect of age on surgical results was compared, and the numerical results and complication rates of endoscopy-assisted carpal tunnel release surgery were compared with the literature. Results: Our endoscopy-assisted carpal tunnel release surgery has made a positive change in satisfaction scores. When we compared our endoscopy-assisted carpal tunnel release surgery with the literature, similar results were obtained, and the complication rates were lower than the literature. The pre- and postoperative values of the scores we used were correlated with each other. Discussion: Endoscopy-assisted carpal tunnel release surgery is an effective treatment method in carpal tunnel syndrome. Clinically, the results are similar to other methods. Problems such as infection, hematoma, scar tenderness, and scarring are lower in this method. The risk of median nerve damage, which is one of the feared complications when performed by experienced hands, is similar to other methods.
