Özet
Amaç: Multiple Myelom tanısı alan hastaların tanı konulduğu zamandaki ölçülen C-reaktif protein (CRP) ve albümin değerlerinin oranının hastalığın prognozuna olan etkisini araştırmayı amaçladık. Materyal ve Metod: Yapılan çalışmaya Ocak 2011 – Ocak 2022 tarihleri arasında İnönü Üniversitesi Turgut Özal Tıp Merkezine başvurusu olan International Myeloma Working Grup (IMWG) kriterlerine göre Multiple Miyelom tanısı almış, tedavisi ve takibi bu merkezde devam etmiş hastaların geriye dönük tanı anındaki değerleri hastane elektronik bilgi yönetim sisteminden (ENLIL) incelenmiştir. Çalışmada elde edilen bulgular değerlendirilirken, istatistiksel analizler için IBM SPSS Statistics 22 programı kullanıldı. Bulgular: Araştırmamız çalışma yaşları 34 ile 87 arasında değişmekte olan, 106'sı erkek ve 81'i kadın olmak üzere toplam 187 olgu ile yapılmıştır. Durie-Salmon evreleme sistemi kullanılmış olup hastaların %50.6'sı Evre 3A, %20.1'i Evre 2, %17.8'i Evre 3B ve %11.5'i Evre 1'dir. Evreler arasında CRP/Albumin oranı ve mortalite görülme açısından istatistiksel olarak anlamlı bir farklılık bulunmamaktadır. Exitus olan olguların CRP/Albumin düzeyleri, sağ olanlardan anlamlı düzeyde yüksek bulunmuştur (p:0.001; p<0.05). Exitus olan olgularda; CRP/Albumin düzeyi ile takip süresi arasında anlamlı bir ilişki bulunmamaktadır. Exitus olan olgularda; yaş ile takip süresi arasında ters yönlü, zayıf düzeyli anlamlı bir ilişki olduğu saptanmıştır. Erkek ve kadınlarda exitus olma oranları arasında anlamlı bir farklılık bulunmamaktadır. Tartışma ve Sonuç: Multiple myelom hastalarında inflamatuvar bir belirteç olan CRP ile Albümin değerinin oranının (CAR değeri) exitus olan olgularda sağ olan olgulardan istatistiksel olarak anlamlı yüksek olduğu görülmüştür. Purpose: We aim to investigate the impact of the ratio of measured C-reactive protein and albumin values at the time of diagnosis on the prognosis of patients diagnosed with multiple myeloma. Materials and Methods: The study included a retrospective analysis of patients diagnosed with Multiple Myeloma according to the International Myeloma Working Group (IMWG) criteria, who had applied to Inönü University Turgut Ozal Medical Center between January 2011 and January 2022, and whose treatment and follow-up were conducted at this center. The values at the time of diagnosis for these patients were examined through the hospital's electronic information management system (ENLIL). In the evaluation of the findings obtained in the study, IBM SPSS Statistics 22 program was utilized for statistical analyses. Result: Our study was conducted with a total of 187 cases, ranging in working age from 34 to 87 years, including 106 males and 81 females. The Durie-Salmon staging system has been used, and %50.6 of the patients are in Stage 3A, %20.1 are in Stage 2, %17.8 are in Stage 3B, and %11.5 are in Stage 1. There is no statistically significant difference between the stages in terms of CRP/Albumin ratio and mortality. The CRP/Albumin levels of the deceased cases were found to be significantly higher compared to those who survived(p:0.001; p<0.05). There is no significant relationship between CRP/Albumin level and follow-up duration in deceased cases. In deceased cases, there is a weak, inverse relationship between age and follow-up duration. The death rate in males and females does not show a significant difference. Discussion and Conclusion: In multiple myeloma patients, the ratio of the inflammatory marker C-reactive protein to albumin (CAR value) was found to be statistically significantly higher in deceased cases compared to the survivors.
