Özet
Amaç: Bu çalışmada kliniğimizde renal kitle nedeni ile nefron koruyucu cerrahi olan parsiyel nefrektomi yapılan hastalar geriye dönük tarandı. Hastalar arasında preoperatif nötrofil/lenfosit oranının nefrometri skorlarından en sık kullanılanı olan PADUA skorlama sistemi ile ilişkisine bakılması amaçlanmıştır. Kriter olarak tümörlerin evresi, boyutu, histolojik derecesi, PADUA skoru, preoperatif ve postoperatif hemoglobin, trombosit ve nötrofil/lenfosit oranları, kanama miktarları alındı. Materyal ve Metod: Çalışmaya 51 hasta ve 52 renal kitle dahil edildi. Bu kitlelerin 5 tanesi laparoskopik, 47 tanesi de açık teknik ile eksize edildi. Hastalara preoperatif ve postoperatif nötrofil/lenfosit oranları, PADUA skorları, kanama miktarları, patolojik tipleri, histolojik dereceleri açısından anlamlılık testi yapıldı. Bulgular: Parsiyel nefrektomi yaptığımız hastaların yapılan istatistiksel karşılaştırılmasında histolojik derece, patolojik tip, PADUA skoru, preoperatif ve postoperatif nötrofil/lenfosit oranları açısından anlamlı farklılık tespit edilmedi. Sadece yaş ile tümör boyutunun arttığı ve preoperatif nötrofil/lenfosit oranı ile kanama miktarının anlamlı olarak korele oldukları tespit edildi. Sonuç: Parsiyel nefrektomi ameliyatı özellikle T1 erken evre tümörlerde güvenle uygulanabilen bir tekniktir. Ancak erken evre tümörlerde preoperatif nötrofil/lenfosit oranı prognostik bir faktör olarak kullanılamayacağını düşünmekteyiz. Aim: In this study, patients who underwent partial nephrectomy through the nephron sparing surgery for renal mass in our clinic were retrospectively screened. We aimed to evaluate patients by preoperative neutrophil to lymphocyte ratio insidence and the relation with PADUA scoring system, which is the widely used nephrometry score. Predictors of tumor stage, size, histological grade, PADUA score, preoperative and postoperative hemoglobin, platelet and neutrophil to lymphocyte ratio, bleeding quantities were taken as criteria. Material and Method: 51 patients and 52 renal masses were included into the study. Of these, 5 and 47 were operated with laparoscopic and open surgery respectively. Significance test was performed in terms of preoperative and postoperative neutrophil to lymphocyte ratios, PADUA scores, bleeding quantities, pathological types and histological grades. Results: No statistically significant difference was found in the histological grade, pathologic type, PADUA score, preoperative and postoperative neutrophil to lymphocyte ratios in the statistical comparison of patients with partial nephrectomy. Only age together with tumor size and preoperative neutrophil to lymphocyte ratio together with bleeding amount significantly correlated. Conclusion: Partial nephrectomy is a technique that can be safely performed especially in T1 early stage tumors. However, we think that preoperative neutrophil / lymphocyte ratio can not be used as a prognostic factor in early stage tumors.
