Özet
Bu çalışmanın amacı, tanımlayıcı bir metod ile Koroner Arter Bypass Greft
(KABG) ameliyatı geçiren hastaların taburculuk sonrası öğrenim gereksinimlerini
belirlemektir.
Bu araştırma Haziran–Aralık 2013 tarihleri arasında ameliyat olan 18 yaş ve
üzerinde olan 171 hasta ile yapılmıştır. Araştırmanın evreni; İnönü Üniversitesi
Turgut Özal Tıp Merkezi Kalp Damar Cerrahisi Kliniğinde KABG ameliyatı
geçirmiş, taburculuğu planlanmış hastalardan oluşmaktadır. Örneklemi ise; yapılan
güç analizinde; küçük etki büyüklüğünde (0.25), 0.95 evreni temsil etme gücüne
sahip 276 KABG ameliyatı olan hastalardan oluşmuştur. Araştırma toplam 171 hasta
ile tamamlanmıştır. Verilerin toplamasında araştırmacı tarafından oluşturulan Hasta
Bilgi Formu (EK-1), Hasta Öğrenim Gereksinimleri Ölçeği ( (HÖGÖ) (EK-2)
kullanılmıştır. Verilerin değerlendirilmesinde SPSS 17 paket programı ve verilerin
analizinde tanımlayıcı istatistikler, t-testi, tek yönlü varyans analizi, kruskal wallis
varyans analizi, testleri kullanılmıştır.
Hastaların öğrenim gereksinimleri toplam puan ortalaması
168.00±25.00, önemlilik düzeyi 3.66 olarak önemli olduğu bulundu. 30-42 yaş
grubundaki hastaların öğrenim gereksinimleri puan ortalamaları ileri yaştaki
hastalardan, bekar olan hastaların ise boşanmış hastalardan daha yüksekti ve
aralarındaki fark önemliydi (sırası ile; p<0.05,p<0.001). Eşi tarafından bakım verilen
hastalar, diğer bakım veren kişilerden, bakım verenin taburculuk sırasında eğitim
alanların, almayanlara göre öğrenim gereksinimleri daha yüksek ve aradaki fark
istatistiksel olarak önemlidir (sırası ile p<0.05, p<0.05 ) .
Bu çalışmada KABG ameliyatı geçiren hastaların iyileşme döneminin büyük
bir bölümünün evde olması nedeniyle taburculuk eğitimi önem kazanmıştır. Hemşire
hastanın taburculuk öncesi ve sonrası öğrenim gereksinimlerini belirleyerek, hasta ve
yakınlarına eğitim yaparak öğrenim gereksiniminlerini azaltabilir.
The aim of this study was to identify learning needs after discharging patients with coronary artery bypass greft (CABG) with a descriptive method. This research was conducted with 171 patients who over 18 years of age, underwent CABG surgery between June to December 2013. The universe of the study was consisted of patients who planned discharge, have undergone CABG surgery in the Department of Cardiovascular Surgery at Inonu University Turgut Ozal Medical Center. The sample, in power analysis that in small effect size (0.25), 0.95 had the power to represent the universe was composed of 276 patients who underwent CABG surgery. Research has been completed with a total of 171 patients. Patient Information Form (Appendix 1) and Patient Education Requirements Scale (Appendix 2) that were created by researchers was used for data collection. Data was evaluated by SPSS 17 software packages and descriptive statistics, t-test, one-way ANOVA, Kruskal-Wallis variance test analysis was used in data analysis. Total mean score of learning needs of patients was 168.00 ± 25.00, significance level with 3.66 was found to be significant. The mean scores of learning needs of patients in the 30-42 age group was higher than elderly patients, single patients was higher than the divorced, and the difference was statistically significant (respectively; p<0.05,p<0.001). When compared the patients' wife with other caregivers and caregivers who educated at discharge with did not educate, they had higher education requirements and the difference was statistically significant (respectively; p<0.05, p<0.05). In this study, discharge education of patients who underwent CABG surgery has gained importance due to spending large part of recovery period at home. Nurse can reduce learning needs by identifying learnings needs of patients before and after discharge, educating patients and patients' relatives.
