dc.description.abstractGenel anestezide kullanılan isofluran ve narkotiklerin troidektomi vakalarında tiroid fonksiyonlarına etkileri biyokimyasal olarak incelendi. Klinik ve laboratuar olarak ötiroid olan 40 hasta iki gruba ayrılarak çalışıldı. İndüksiyon I. grupta 5 mg/kg sodyum tiopentan ve 0.6 mg/kg atrakuryum verilerek sağlandı. İdame anesteziye % 40 O2, % 60 N2O ve % 1,5 isofluranla devam edildi. II. grupta indüksiyon 0,2 mg/kg dehidrobenzperidol, 40 pg/kgfentanil ve 0,6 mg/kg atrakuryumla sağlandı. İdamede % 40 02,% 60 N2O ve 20-30 dakika aralıklarla 0,05 mg fentanil verildi. Hormonların tespiti için , kontrol, indüksiyon sonrası, 1 ve 2. saatte ve postoperatif 24. saatte kan örnekleri alındı. I. grupta T3 seviyesi 1. saatte (p<0.05) düştü. II. grupta ise indüksiyonda (p<0.001) ve 1. saatte (p<0.05) yükseldi. T4 ve TSH da her iki grupta da değişiklik (p>0.05) olmadı. Her iki grupta da serbest T3 (p<0.05) düştü. Serbest T4 I. grupta 1., 2. ve 24. saatte yükseldi (p<0.05). Sonuç olarak troid hormon değerleri sınırda veya yüksek olan vakalarda isofluran anestezisinin tercih edilmesinin daha iyi olacağı kanaatine vardık.en_US
dc.description.abstractThe effects of isoflurane and narcotics used in general anaesthesia on thyroid hormones were studied biochemically. 40 patients who were euthyroid both hormonally and clinically divided to two groups. In 1st group; induction was provided by using 5 mg/kg Na thiopenthal and 0.6 mg/kg atracrium. Anaesthesia has been maintained with 40% O2, 60% N2O, and 1.5% isoflurane. Second group was induced with 0.2 mg/kg dehydrobenzperidol, 40 pgr/kg fentanyl and 0.6 mg/kg atracrium, and maintained with 40% O2, 60% N2O, and 0.05 mg fentanyl given at 20-30 minutes intervals. Hormone levels were obtained preoperatively, immediately after induction, and postoperative 1st, 2nt, and 24th hours. T3 levels decreased at postoperative 1st hours in the first group (p<0.05) and elevated after the induction and 1st hour in the second group (p<0.05). T4 and TSH levels did not change in both groups (p>0.05). Free T3 decreased in both groups (p<0.05). Free T4 elevated at 1st, 2nt, and 24th hours in first group (p<0.05). We concluded that isoflurane anaesthesia is a better choice for patients with high thyroid hormone levels are high or at the upper limits.en_US
