Subjektif tinnitus şikayeti olan normal işitmeye sahip bireylerde elektrofizyolojik yanıtların incelenmesi

Özet
Amaç: Bu çalışmanın amacı normal işitmeye sahip subjektif tinnitusu olan bireylerin etkilenim süresinin elektroakustik yanıtlar, elektrofizyolojik yanıtlar, bilişsel yetenekler ve günlük yaşam kalitesi üzerindeki etkisini değerlendirmektir. Materyal ve Metot: Çalışma, İnönü Üniversitesi Turgut Özal Tıp Merkezi Odyoloji kliniğinde Aralık 2023- Ekim 2024 tarihleri arasında normal işitmeye sahip subjektif tinnituslu gönüllü bireyler dahil edilerek gerçekleştirilmiştir. Bireyler tinnitus etkilenim sürelerine göre kısa dönem, uzun dönem olmak üzere gruplandırılmıştır. Çalışmaya dahil edilme kriterlerini karşılayan kısa dönem grubuna 17 birey, uzun dönem grubuna 17 birey ve kontrol grubuna 17 birey dahil edilmiştir. Bireylere TEOAE, DPOAE, kontralateral supresyon, ABR testleri uygulanmış; THE, KUİK ölçeklerini doldurmaları talep edilmiştir. Bulgular: Çalışma bulgularına göre TEOAE test sonucunda 1400 Hz frekansında uzun dönem grubu kontrol grubuna göre anlamlı derecede düşük elde edilmiştir (p<0.05). DPOAE test sonucunda 2000 Hz, 2800 Hz, 4000 Hz frekanslarında uzun dönem grubunda kontrol grubuna kıyasla anlamlı derecede düşük sonuç elde edilmiştir (p<0.05). Kontralateral supresyon ve ABR test sonuçlarında gruplar arasında anlamlı bir fark izlenmemiştir (p>0.05). KUİK ölçeği skorlarında uzun dönem grubu kontrol grubuna göre anlamlı derecede düşük elde edilmiştir (p<0.05). THE ölçek skorlarında uzun dönem grubunda kısa dönem grubuna kıyasla anlamlı derecede yüksek sonuçlar izlenmiştir (p<0.05). THE ve KUİK ölçekleri arasında negatif bir korelasyon izlenmiştir. Sonuç: Elde edilen sonuçlar doğrultusunda tinnitus etkilenim süresindeki artışın dış tüylü hücrelerden alınan yanıtları etkilediği sonucuna varılmıştır. Ölçek bulgularına göre etkilenim süresi arttıkça bilişsel düzeyde etkilenimin arttığı ve hayat kalitesinin düştüğü gözlemlenmiştir.
Aim: The aim of this study was to evaluate the effect of exposure time on electroacoustic responses, electrophysiological responses, cognitive abilities and daily life quality of individuals with subjective tinnitus and normal hearing. Material and Method: The study was conducted at the Audiology Clinic of Inonu University Turgut Ozal Medical Center between December 2023 and October 2024 by including volunteer individuals with subjective tinnitus with normal hearing. Individuals were grouped as short-term and long-term according to their tinnitus exposure duration. 17 individuals who met the inclusion criteria were included in the short-term group, 17 individuals in the long-term group and 17 individuals in the control group. TEOAE, DPOAE, contralateral suppression, ABR tests were applied to the individuals; they were asked to fill out the THI and SSQ scales. Results: According to the study findings, the TEOAE test result was significantly lower in the long-term group compared to the control group at 1400 Hz frequency (p<0.05). The DPOAE test result was significantly lower in the long-term group compared to the control group at 2000 Hz, 2800 Hz, and 4000 Hz frequencies (p<0.05). No significant difference was observed between the groups in the contralateral suppression and ABR test results (p>0.05). The long-term group was significantly lower in the SSQ scale scores compared to the control group (p<0.05). The THI scale scores were significantly higher in the long-term group compared to the short-term group (p<0.05). A negative correlation was observed between the THI and SSQ scales. Conclusion: According to the obtained results, it was concluded that the increase in the tinnitus exposure time affected the responses received from the outer hair cells. According to the scale findings, it was observed that as the exposure time increased, the cognitive level increased and the quality of life decreased.
