Özet
Öz: Daha az ve yavaş ateroskleroz gelişimi ile sonuçlanan biyolojik özelliklerinden dolayı arteryel greftlerin açıklık oranları venöz greftlerden daha yüksektir. Bu çalışmada radial arter kullanarak tam arteryel revaskülarizasyon deneyimimizi sunmaktayız. Eylül 2001 ile Nisan 2002 tarihleri arasında koroner greft olarak tek (25 hasta) veya bilateral (22 hasta) radial arter kullandığımız çok damar hastalığına sahip 47 olguya total arteryel myokardiyal revaskülarizasyon uyguladık. Tüm hastalarda sol ön inen arter revaskülarizasyonunda sol internal mammarian arter kullandık. Radial arter; olguların 37'sinde tek distal anastomoz için, 10'unde ise sequential olarak kullanıldı. Olguların 39'u erkek ve 8'i kadın olup ortalama yaş 54.09±11.4 (30 - 75) idi. Ortalama distal anastomoz sayısı 2.69±0.84 (2 - 5 ) olarak gerçekleştirildi. Operatif mortalite oranı %4.3 idi. Erken postoperatif dönem problemsizdi ve 1-6 aylık takipte olgularımız semptomsuz olarak yaşamlarını sürdürmektedirler. Hiç bir hastada radial arter çıkarılmasına bağlı bir komplikasyon tespit edilmedi. Tek veya bilateral radial arter kullanılarak tam arteryel revaskülarizasyon basit ve güvenli bir şekilde gerçekleştirilebilir ve böylece safen ven çıkarılmasına bağlı oluşabilecek potansiyel komplikasyonlardan kaçınılmış olmaktadır.
Abstract: The patency rate of arterial grafts is higher than that of venous grafts because of their biological characteristics that results in slower and lesser development of arteriosclerosis. In this study we present our experience with complete arterial revascularization using radial artery. We studied 47 consecutive patients with multi-vessel coronary artery disease who had a single RA (25), or bilateral RA (22) as coronary grafts, from September 2001 to April 2002. All the patients had left internal thoracic artery for revascularisation of left anterior descending coronary artery; radial artery was used as individual in 37 patients and used as sequential grafts in 10. There were 39 male and 8 female patients with a mean age of 54.09±11.4 years (30 to 75 years). Mean distal anastomosis number was 2.69± 0.84 (2 -5). Perioperative mortality rate was 4.3%. All has an uneventful early postoperative recovery and are doing well during the period of 1 to 6 months postoperatively. There was no complication related to RA harvesting. Total arterial myocardial revascularisation using unilateral or bilateral RA can be performed simply and safely and also avoided potential complication due to harvesting saphenous vein.
