Gluteal compartment syndrome is a rather rare syndrome often leading to severe sequelae, sepsis, renal failure, and even death due to delayed diagnosis. Establishing early diagnosis is essential to prevent complications associated with ischemia. In this article, we report a 56-year-old male patient who developed gluteal compartment syndrome after incisional hernia and nephrectomy surgery in lateral decubitus position. Gluteal muscle insufficiency developing after fasciotomy and Trendelenburg gait improved within two years.en_US
