Oftalmik cerrahide subtenon ve retrobulber anestezinin prospektif olarak karşılaştırılması

| dc.description.abstract | Öz: Çalışmamızda, son yıllarda geliştirilen subtenon anestezi tekniğinin oftalmik cerrahideki etkinli¬ğinin ve güvenirliğinin prospektif kontrollü bir çalışmayla retrobulber anesteziyle karşılaştırmalı olarak değerlendirilmesi amaçlandı. Bu amaçla, kliniğimizde ekstrakapsüler veya fakoemulsifikasyon yöntemiyle katarakt ekstraksiyonu ya da trabekülektomi gerçekleştirilen 40 olgunun 40 gözü preoperatif randomize olarak subtenon ve retrobulber anestezi gruplarına ay¬rıldı. Subtenon anestezi, inferotemporal veya inferonazal kadranlardan özel subtenon kanülü a-racılığıyla retroekvatoryal subtenon alana anestetik enjeksiyonu yapılarak gerçekleştirildi. Subtenon anestezi grubunda anestetik uygulanımı sırasındaki ağrı miktarı retrobulber gruba gö¬re anlamlı derecede az bulunurken (p=0.022), her iki grupta operasyon sırasındaki, postoperatif dönemdeki ağrı miktarı ve komplikasyonlar arasında istatistiksel anlamlı fark bulunmadı. Retrobulber yöntem glob akinezisinde istatiksel olarak anlamlı oranda daha etkin bulunurken (p=0.0001) subtenon teknikle de belirgin olarak az anestetik madde kullanılarak (p<0.0001) cerra¬hi için yeterli oküler anestezi sağlandığı belirlendi. Subtenon anestezi tekniği oftalmik cerrahide etkili ve retrobulber anesteziye bağlı olası komplikasyonları önleyebilecek güvenilir bir alternatif olarak görünmektedir. | en_US |
| dc.description.abstract | Abstract: Prospective Comparison of Sub-Tenon and Retrobulbar Anesthesia in Ophthalmic Surgery In this study, the efficacy and the safety of sub-Tenon's anesthesia technique in ophthalmic surgical procedures are compared with retrobulbar anesthesia in a prospective controlled study. Forty eyes of 40 cases on which cataract extraction or trabeculectomy performed were randomized as to whether they would receive retrobulbar or sub-Tenon's anesthesia. A curved cannula designed for sub-Tenon's anesthesia was utilized to deliver anesthetic into sub-Tenon's space from inferotemporal or inferonasal quadrant in sub-Tenon's group. While patient discomfort was significantly lower during significant difference was found in intraoperative ocular anesthesia efficacy, postoperative discomfort or complications between the two groups. Globe akinesia was significantly better (p=0.0001) in the retrobulbar group but a sufficient ocular anesthesia for the procedures was also achieved in sub-Tenon's group using significantly lower (p<0.0001) anesthetic volume. Sub-Tenon's anesthesia technique seems to be a safe and effective alternative in ophthalmic surgery that may prevent probable complications due to retrobulbar anesthesia. | en_US |
