dc.description.abstractAkciğer transplantasyonunda donör azlığı ve rejeksiyon çok önemli sorunlardır. Ebeveynden lob transplantasyonu bu sorunlara çözüm olabilir. Merkezimiz hayvan laboratuarında bir anneden iki yavruya birer akciğer lobu ve bir anneden bir yavruya bir akciğer lobu transplantasyonu gerçekleştirilmiştir. Ameliyatlar kardiyopulmoner bypas kullanılmadan, santral venöz basınç, invaziv arteriyel basınç ve oksijenasyon takibi ile gerçekleştirilmiştir. Sadece preoperatif tek doz steroid dışında immünsupresif medikasyon yapılmamıştır. Anneye sol pnömonektomi yapılıp, iki lob ayrılarak annenin iki yavrusuna sol pnömonektomiyi takiben birer lob, sol akciğer yerine transplante edilmiştir. Bir diğer anneye ise sol alt lobektomi ve yavrusuna sol pnömonektomi yapılmış, ve anneden çıkartılan lob pnömonektomize yavruya sol akciğer olarak tranplante edilmiştir. Pnömonektomi yapılan anne postoperatif anestezi komplikasyonu nedeniyle eksitus olmuş, birer lob transferi yapılan yavru köpekler postoperatif 9 ve 17 saat yaşamış ve solunum yetmezliği nedeniyle kaybedilmiştir. Lobektomi yapılan ikinci annede ise postoperatif sorun olmamış fakat lob alıcısı yavru trasplantasyonun altıncı günü akut alveoler hasar nedeniyle kaybedilmiştir. Köpeklerde akciğer lob transplantasyonu teknik olarak mümkündür. Canlıdan lob transplantasyonu özellikle pediatrik yaş grubundaki akciğer donör azlığı sorununa bir çözüm olabilir.en_US
dc.description.abstractDonor shortage and rejection are very important problems in lung transplantation. Lobar transplantation from parents may be a solution for these problems. Two lobar transplantations from a mother dog to two puppies and one lobar transplantation from annother mother to one puppy were performed in our animal laboratory. Operations were performed without cardiopulmonary bypass, and central venous pressure, invasive arterial pressure and oxygenation were monitorized. Any immune suppresive medication was not used except one dose of preoperative corticosteroid. The mother underwent left pneumonectomy, then two lobes were prepared and transplanted to the left hemithoraces of two pneumonectomized puppies as left lungs. Left lower lobectomy was performed in the other mother dog and that lobe was transplanted to her pneumonectomized puppy as left lung. The mother underwent pneumonectomy died postoperatively because of anesthetic complications and each of two puppies received pulmonary lobes lived for 9 and 17 hours postoperatively and died because of respiratory insufficiency. The second mother underwent lobectomy survived without any complication but her puppy received left lower lobe died on postoperative day six from acute alveolar damage. Pulmonary lobar transplantation is technically feasible in dogs. Living-related lobar transplantation may be a solution for donor shortage problem in pediatric age group.en_US
