Özet
Öz: İntravitreal deksametazon implant (Ozurdex®, Allergan, USA) retina ven tıkanıklıkları (RVT) yanında diyabet ve üveit gibi nedenlere bağlı maküler ödem tedavisinde günümüzde sıkça kullanılmaktadır. Bu uygulamanın en sık görülen yan etkileri göz içi basınç artışı ve katarakt gelişimidir. Bu çalışmada intravitreal deksametazon implant uygulamasının nadir bir komplikasyonu olarak retinal ven tıkanıklığına bağlı maküla ödemi nedeniyle intravitreal uygulanması planlanan ancak implantın yanlışlıkla lens içine yerleştiği iki olguyu sunmak amaçlandı. Lens içine enjeksiyon nedenleri arasında hekim dikkatsizliği, hasta uyumsuzluğu, lokal anestezi tipi, enjeksiyon sahasının limbusa uzaklığı, hipotoni, skleral zayıfl ık, tekrarlayan enjeksiyonlara bağlı lensi tutan zonüllerde hasar oluşması ile lens pozisyonun değişmesi sayılabilir.Bu nadir komplikasyondan kaçınmak için özellikle implant uygulamaları sırasında enjeksiyon tekniğinin iyi bilinmesi önem taşımaktadır.
Abstract: An intravitreal dexamethasone implant (Ozurdex®, Allergan, USA) is currently commonly used for the treatment of macular edema due to causes such as retinal vein occlusion (RVO) and also diabetes and uveitis. We present two cases where a dexamethasone implant planned to be placed intravitreally for macular edema due to retinal vein occlusion was instead placed into the lens body by mistake as a rare complication of intravitreal dexamethasone implant injection. Possible causes of the inadvertent injection into the lens body include physician carelessness, patient incompliance, type of local anesthesia, the distance of the injection area to the limbus, hypotonia, weakness of the sclera, damage in the zonules holding the lens due to repeated injections, and a change in lens position. A good knowledge of injection technique, especially during procedures involving implants, is required to avoid this rare complication.
