Öğe Acute inter hemispheric subdural hematomas report of nine cases(Journal of Neurosurgical Sciences, 2001) Şenel, Alparslan; Çokluk, Cengiz; Önder, Arif; İyigün, Ömer Lütfi; İncesu, LütfiNine male patients with acute interhemispheric subdural hematoma (ISH) are presented. The etiologic factor was trauma for all patients (traffic accident and falling down). The young adult cases and one child had bad prognosis due to severe clinical findings and high mortality. The asymptomatic patients were treated conservatively. In this report, we discussed etiologic factors, presentation of age groups, whether an ISH progresses to a chronic convexity subdural hematoma (SH), and real mortality rates for ISH with relevant literature knowledge. As a result, ISHs can present in all age groups including shaken babies, severely injured young adults as well as low velocity trauma striken elderly patients (especially those under anticoagulant medication). We believe that an acute ISH does not change to chronic convexity SH; in fact they present as simultaneous acute thin convexity SH and acute ISH. It is also concluded that in contrast to previous literature ISH and acute SH patients of similar neurological status have similar mortality rates.Öğe Akut subdural hematomlar klinik analiz(Atatürk Üniv. Ellidiirdii (%83.1)erkek, 1l'i (%16.9)kadm dnsten olan 65 hastamn, 34'ii (%52.3)dii~me, 27'si (%41.5)trafik kazasl ve 4'ii (%6.2) ate~lisilah yaralanmasl sonucu başvurmuşlardr. Olgulardan 18'inde (%27.7)klinik seyir iiliimle sonu9anml~tI. blen olgulann 15'inde (%83.3)aglr niirolojik defisit, 5'inde (%27.7)agrr toraks patolojisi, 2'sinde (%11.1)intrakraniyal kitle yard!. Hastalann 6'SI (%9.2)ilk 72 saat içinde ölmüşlerdi. Yalmz spinal yaralanmasl olan olgular da dahi yiiksek mortalite ve morbiditeye sahip olan servikal spinal yaralanmalarda, spinal yaralanmaya ilave olan diger sistemlere ait travmatik komplikasyonlar bu oram daha da yiikseltrnektedir. There is a confusion about the inferior border of the odontoid. The aims of this clinical study were to describe the inferior border of the odontoid process based on the remnant of dentocentral synchondrosis in adults, and the estimation of the odontoid/body ratio in pediatric and adult ages. Sixty-six cases were included for this study. Forty-four of them were in adult ages and the remaining 22 of them were in pediatric ages. The region of occiput, C1, C2, and C3, was examined with the magnetic resonance imaging (MRI) in all cases. The length of the odontoid process was estimated by using radiological images of MR from the tip of the odontoid to the remnant of dentocentral synchondrosis. The portion located under the level of synchondrosis was consid ered as the body of C2. The average length of the odontoid was 18. 6 mm in pediatric and 21. 3 mm in adult cases. In adult females, the length of the odontoid process (19. 1 mm in length) was smaller than those of adult males (23. 6 mm in length). The average ratio of odontoid/body was two in pediatric and 1.8 in adult cases. This study demonstrated that the neck of the odontoid segment at the level of superior articulating facets is not the synchondrosis between the odontoid process and the body of C2. The synchondrosis is located well below the level of superior articulating facets. It can be demonstrated with sagittal and coronal images of MR in both of pediatric and adult individuals.Öğe Chrysin prevents brain damage caused by global cerebralischemia reperfusion in a C57BL J6 mouse model(Turkısh journal of medıcal scıences, 2016) Durak, Mehmet Akif; Öztanır, Mustafa Namık; Türkmen, Neşe Başak; Çiftçi, Osman; Taşlıdere, Aslı; Tecellioğlu, Mehmet; Önder, ArifThe present study investigated the neuroprotective effects of chrysin (CRS) following global cerebral ischemia and reperfusion (I/R) in a C57BL/J6 mouse model. Materials and methods: A total of 40 mice were equally divided into four groups: (1) sham-operated (SH = control), (2) global cerebral I/R (I/R), (3) CRS, and (4) CRS + I/R. In the I/R group, the bilateral carotid arteries were clipped for 15 min and the mice were treated with vehicle (corn oil) for 10 days. In the CRS group, CRS (50 mg/kg) was given for 10 days without carotid occlusion. In the CRS + I/R group bilateral carotid arteries were clipped for 15 min and the mice were also treated with CRS (50 mg/kg) for 10 days. All of the rats were sacrificed under anesthesia on day 10, and neurodegenerative histological changes in the brain and tissue levels of oxidants and antioxidants were evaluated. Results: CRS treatment significantly reversed the oxidative effects of I/R and inhibited the development of neurodegenerative histopathologies. In the CRS + I/R group, the decrease in TBARS levels and increase in GSH levels were similar to those in the SH group. Conclusion: Treatment with CRS can positively affect the neural system of mice and it can be used for the treatment of global cerebral I/R.Öğe Clinical analysis of cerebral abscesses(Zent Bl Neurochir, 1988) Aydın, İsmail Hakkı; Aladağ, Mehmet Arif; Kadıoğlu, Hakan Hadi; Önder, Arif42 patients with cerebral abscesses were treated from 1979 until the end of the first six months of 1987 at the Neurosurgical Department of the Faculty of Medicine, Atatürk University, Erzurum, Turkey. The present study aims at studying these patients to determine the factors affecting on prognosis. The patients were studied for the duration of complaints, pre-operational condition, the location of the abscesses, the microorganisms responsible for them, and the techniques of operation applied, by taking their sex and age into consideration. All of the cases were treated operatively. Of these, 18 were treated by excision and 24 were applied drainage, whose percentage was 43 and 57 respectively, 85% of the cases were under 60 years of age. The rate of mortality of the cases with cerebral abscesses was 19%. This rate was between 15% and 37.5% in the cases of several age groups whose complaints lasted less than two months. This rate reached 100% with a sudden increase in the cases who had a duration of complaints over four months. The rate of mortality was 6.2% in the cases with a clear conscious and cooperative ability while it had a significant increase in the patients in pre-coma or coma to go up to 50% and 75% respectively. No correlation was observed between the rate of mortality and the age of the patients but the location and extension of the abscesses had an effect on this rate. The abscesses, covering more than one lobe, had a mortality rate of 31.2%, while the ones with parietal location caused no single death. To conclude, we can say, based on this study, that the age and sex of patients and the operative technique have no effect on the rate of mortality while it is obviously influenced by the duration of the complaints and the pre-operative condition. Tedavi verilen ve verilmeyen gruplar arasl farklar istatiksel yonden anlamh olarak bulundu. Bu deneysel çalIşmaOA tedavisinin SAK'tan sonra serebral vazospazmll1 onlenmesinde etkili oldugunu duşundiirmektedir. Yan etki profilinin oldukça duşuk olmasl ve subkutan yolla kullal1llabilmesi diger avantajlandu. MaterIal and Methods: Forty Sprague Dawley rats were used and divided into the 2 groups as control and study groups. In the control group, the rats did not suffer noise injury. The rats were exposed to impulse noise at 20 minute durations at 10 times each day for one month in study group. Then, the rats were sacrificed and the choroid plexuses were examined histologically. The number of cells was counted and the cells were analyzed. Results: There were clear signs of nuclear condensation and cell body shrinkage, suggesting the presence of apoptosis. Severe desquamation of villus and the cell loss were observed in the study group. The numbers of the normal cells decreased, and the number of apoptotic cells increased significantly (p<0.05). ConclusIon: Impulse noise causes apoptotic death of epithelial cells in the choroid plexus, decrease the normal cells and increase the apoptotic cells.Öğe The effect of very early cisternal irrigation on basilar artery spasm after SAH In the rat model(Acta Neurochir, 1991) Aydın, İsmail Hakkı; Önder, ArifThe authors have investigated the effect of very early irrigation of the cerebrospinal fluid (CSF) space in the haemorrhage rat model of vasospasm. Fifteen rats had basilar cistern irrigation with physiological saline for 3 hours after subarachnoid haemorrhage (SAH), and fifteen control rats had subarachnoid haemorrhage without irrigation of clot. The changes in basilar arteries diameters were determined by angiograms obtained from the rats. The post haemorrhage anglograms showed significant basilar artery spasm in both groups (P ~< 0.0005, t-test). However in the last angiogram the basilar artery diameter was found to have the same value measured before subarachnoid haemorrhage in the irrigation group whereas no obvious change was observed in the control group. In the irrigation group the mean diameter of the basilar artery in the last angiogram was 0.412ram. (0.30ram to 0.50mm). None of the animals, treated by cisternal irrigation, showed angiographic vasospasm while the latter group did (P ~< 0.0005). Animals treated with physiological saline irrigation had a median clot grade of 0.40 (range grade 0 to 2); control rats had a median grade 2.86 (range grade 1 to 4, P < 0.001, Mann-Withney U test), on the brain stem, indicating significant reduction of clot by lavage. In conclusion, performance of experimental physiological saline irrigation at a very early time after subarachnoid haemorrhage prevents the arteriographic and morphological changes of both acute and late vasospasms.
